通便承气方穴位贴敷治疗椎体骨质疏松性压缩骨折患者腹胀便秘的临床观察

摘　　要：【目的】观察通便承气方穴位贴敷治疗椎体骨质疏松性压缩骨折患者腹胀便秘的临床疗效。【方法】将90例伴发腹胀便秘的椎体骨质疏松性压缩骨折患者随机分为试验组46例和对照组44例。试验组患者给予通便承气方药贴贴敷于神阙、天枢、足三里等穴治疗,对照组患者给予乳果糖口服治疗,至肠蠕动恢复、肛门排气通便为止,疗程不超过5 d。观察2组患者的首次排气、排便时间及治疗前后临床症候评分的变化情况,评价2组患者的临床疗效和安全性。【结果】(1)治疗72 h后,试验组的总有效率为89.1%(41/46),对照组为79.5%(35/44),试验组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗24 h和48 h后,2组患者的临床症候评分均较治疗前明显降低(P<0.05),且试验组的降低作用均明显优于对照组,差异均有统计学意义(P<0.05)。(3)治疗后,试验组患者的首次排气和排便时间均较对照组明显缩短,差异均有统计学意义(P<0.05)。(4)治疗期间,2组患者均无明显不良反应发生。【结论】通便承气方穴位贴敷治疗椎体骨质疏松性压缩骨折患者的腹胀便秘,疗效确切,可明显缩短治疗时间,减轻患者腹胀、便秘症状,且安全性较高。Objective To observe the clinical efficacy of acupoint application of Tongbian Chengqi Prescription in the treatment of abdominal distension and constipation in the patients after osteoporotic vertebral compression fracture. Methods Ninety patients with abdominal distension and constipation after osteoporotic vertebral compression fracture were divided into trial group(46 cases)and control group(44 cases). The trial group was treated with acupoint application of Tongbian Chengqi Prescription at the points of Shenque(CV8), Tianshu(ST25)and Zusanli(ST36). The control group was given oral use of lactulose till the recovery of intestinal peristalsis and the occurrence of anal exhaust and defecation,and the course of treatment was less than 5 days. The time for the first anal exhaust and defecation after the fracture was recorded, and the changes in the scores of traditional Chinese medicine(TCM)syndrome before and after treatment between the two groups were compared.After treatment,the clinical efficacy and safety of the two groups were evaluated. Results(1)Seventy-two hours after treatment,the total effective rate of the trial group was 89.1%,and that of the control group was 79.5%. The clinical efficacy of the trial group was superior to that of the control group,and the difference was statistically significant(P<0.05).(2)Twenty-four and 48 hours after treatment,the TCM syndrome scores in the two groups were obviously decreased in comparison with those before treatment(P<0.05),and the decrease in the trial group was superior to that in the control group(P<0.05).(3)After treatment, time for the first anal exhaust and defecation in the trial group was much shortened in comparison with that in the control group,and the difference was statistically significant(P<0.05).(4)During the trial, no obvious adverse reaction occurred in the two groups.
