加味小柴胡汤治疗小儿支原体肺炎临床研究

摘　　要：目的:观察加味小柴胡汤治疗小儿支原体肺炎的疗效及对患者炎症反应、免疫功能及不良反应的影响。方法:选取2017年6月^2018年7月收治的小儿支原体肺炎患者102例,采用随机数表法将患者分为两组,每组51例。对照组患者接受大环内酯类抗生素治疗,观察组患者在对照组的基础上服用加味小柴胡汤,10天后比较两组患者的疗效、中医症候积分、临床指标改善时间、炎症反应指标、免疫指标及不良反应发生情况。结果:观察组患者治疗总有效率及治疗后CD3^+、CD4^+、CD4^+/CD8^+较对照组高(P<0.05);治疗后观察组患者中医症候积分、CRP、IL-6、TNF-α水平及不良反应发生率较对照组低(P<0.05);观察组患者发热、咳嗽、喘息症状消失及胸片恢复时间均较对照组明显缩短(P<0.05),两组患者肺部啰音消失时间无显著性差异(P>0.05)。结论:加味小柴胡汤治疗小儿支原体肺炎可提高疗效,改善中医症候,提高免疫功能,降低机体炎症反应及不良反应发生率,促进患者康复。Objective:To investigate the curative effect of using Modified Xiaochaihu Decoction in the treatment of children with mycoplasma pneumonia and the effects on inflammatory response,immune function and adverse reactions.Methods:102children with mycoplasma pneumonia treated from June 2017to July 2018were selected.They were divided into two groups by the random number table method with 51cases in each group.Patients in the controlled group were treated with Macrolide Antibi-otics while patients in the observation group were treated with Modified Xiaochaihu Decoction on the basis of the controlled group.10days later,the curative effect,TCM symptom scores,improvement time of clinical indicators,inflammatory response indica-tors,immune indexes and occurrence of adverse reactions were compared between the two groups.Results:The total response rate,CD3^+,CD4^+and CD4^+/CD8^+in the observation group after treatment were higher than those in the controlled group(P<0.05).After treatment,the TCM syndrome scores,levels of CRP,IL-6and TNF-αand the incidence of adverse reactions in the observation group were lower than those in the controlled group(P<0.05).The disappearance time of fever,cough and wheezing symptoms and chest X-ray recovery time were significantly shorter in the observation group than in the controlled group(P<0.05).There was no significant difference in the disappearance time of lung rale between the two groups(P>0.05).Con-clusion:Modified Xiaochaihu Decoction can improve the curative effect of children with mycoplasma pneumonia,relieve TCM symptoms,improve immune function,reduce the incidence of inflammatory reaction and adverse reactions,and promote the re-covery of patients.
