摘 要:目的探讨表面肌电图(sEMG)监测下动态关节松动术用于治疗骨折术后肘关节活动受限的效果。方法选取8例门诊就诊的单侧肘关节骨折术后患者,均有不同程度肘关节活动受限,先行磁场疗法、中频电、超声波治疗及蜡疗,然后在sEMG监测下行动态关节松动术,最后冰敷冷疗,共治疗8周。均在治疗前后进行肘关节总活动度评估及肘关节MaYo功能评分,分别采集双侧肘关节屈曲及伸展至最大角度时,肱二头肌和肱三头肌的平均振幅值(AEMG),且对患侧与健侧AEMG的比值进行比较。结果治疗后,8例患者中7例肘关节总活动度221°~272°,1例190°;肘关节MaYo评分优1例,良6例,可1例,优良率为87.5%。治疗后,肱二头肌及肱三头肌患侧与健侧AEMG的比值均较治疗前明显下降(P<0.001)。结论常规康复治疗的同时在sEMG监测下行动态关节松动术,对肘关节骨折术后导致的活动度受限有明显的治疗效果。Objective To investigate the effect of Mulligan’s mobilization with movement technique under surface electromyography(sEMG)monitoring on elbow joint motion limitation after fracture operation.Methods Eight patients with elbow joint motion limitation after operation for unilateral elbow fracture firstly underwent magnetic therapy,intermediate frequency electrical therapy,ultrasound treatment and paraffin treatment.Then,patients were treated with Mulligan’s mobilization with movement technique under sEMG monitoring followed by cold compress therapy.The total elbow joint activity and MaYo elbow function score were detected before and after 8 weeks of treatment.In addition,the average electromyography(AEMG)amplitudes of biceps and triceps were determined at bilateral elbow flexion and maximum extension.Furthermore,the affected side-to-healthy side AEMG ratio was measured.Results After treatment,the total elbow joint activity was 221-272°in 7 patients,and 190°in 1 patient.The MaYo elbow function was excellent in 1 case,good in 6 cases,and fair in 1 case,with an excellent and good rate of 87.5%.The affected side-to-healthy side AEMG ratio significantly decreased in biceps and triceps after treatment(P<0.001).Conclusion Routine rehabilitation treatment combined with Mulligan’s mobilization with movement technique under sEMG monitoring has obvious therapeutic effect on elbow joint motion limitation after fracture operation.
