钛网植入椎体成形术与PKP治疗脆性骨折疗效比较

摘　　要：目的比较经皮钛网植入椎体成形术与经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗胸腰椎脆性骨折疗的临床疗效。方法回顾性分析2015-2019年收治的126例胸腰椎脆性骨折患者,66例行传统PKP治疗(PKP组),60例行钛网植入椎体成形术(钛网组),分别记录两组患者术中所用手术时间、透视次数、骨水泥注入量、手术并发症、椎体高度复原及保持情况、患者腰背部疼痛缓解情况。结果与单纯PKP组相比,钛网组在骨水泥注入量、椎体高度复原情况、术后椎体高度保持情况、长期临床疗效等方面差异均有统计学意义(P<0.05),两组术中透视次数,手术时间差异无统计学意义(P>0.05)。结论与PKP组相比,钛网植入椎体成形术能够更好恢复椎体高度,并且术后能较好维持椎体高度,远期疗效优于PKP手术。Objective To compare two clinical retults of percutaneous titanium mesh implantation andpercutaneous kyphoplasty(PKP)for osteoporotic comparessions fracture.Methods A retrospective analyze wasconducted on 126 patients who received surgical treatments during 2015 to 2018. Among them,66 patients(namedPKP Group)reveived PKP,whereas the other 60 patients(named Titanium mesh Group)accepted percutaneoustitanium mesh implantations. We had separate records on the following parameters:the operation time,the time of X-rayfluoroscopy,the injection amount of medical bone cement,complications,the height of vertebral body andback pain operation treatment currently tracking in long term and so on.Results A statistical effort(P < 0.05)has been proven on Titanium mesh Group from the height keeping of vertebral body and the treatment of back painto the height of vertebral bodies due to the long-term tracking after treatments. There had a statistical differences(P < 0.05)on the height keeping of vertebral body and the treatment of back pain by a long-term follow-up. Thedisparities of operation time and the times of X-ray do not work out any statistical meaning.Conclusions In thesecases,the Tianium mesh Implantation Group is better than PKP Group when dealing with the following key data:restoring and maintaining the height of the vertebral body,and the effects in long term.
