摘　　要：目的观察针刺率谷穴配合穴位注射治疗神经性耳鸣的临床疗效。方法将220例神经性耳鸣患者随机分为对照组和观察组,每组110例。对照组给予常规西药治疗,观察组在对照组治疗基础上给予针刺率谷穴配合穴位注射治疗。观察两组治疗前后的耳鸣主要症状体征评分、耳鸣响度评分和耳鸣残疾量表(THI)评分变化,并比较两组临床疗效。结果治疗后,两组患者的耳鸣发生环境、对睡眠影响、持续时间、对生活工作影响、对情绪影响和主观感受6个维度评分显著降低(P<0.05),观察组低于对照组(P<0.05)。治疗后,两组耳鸣响度评分和THI评分降低(P<0.05),不同时间组内和组间比较,差异有统计学意义(P<0.05)。观察组的总有效率为95.5%,显著高于对照组的80.0%(P<0.05)。结论针刺率谷穴配合穴位注射治疗神经性耳鸣疗效显著,可有效减轻患者的耳鸣主要症状体征,降低耳鸣响度,改善耳鸣残疾程度。Objective To observe the clinical efficacy of acupuncture at Shuaigu(GB8)plus acupoint injection in treating intractable tinnitus.Method A total of 220 patients with intractable tinnitus were randomized into a control group and an observation group,with 110 cases in each group.The control group was given routine Western medication,based on which the observation group was given acupuncture at Shuaigu(GB8)plus acupoint injection.The Posttreatment changes in the tinnitus main symptom and sign scores,tinnitus loudness score and Tinnitus Handicap Inventory(THI)score were observed in the two groups,and the clinical efficacy was compared between the two groups.Result The scores of 6 domains decreased significantly after treatment in the two groups,including environment triggering tinnitus,influence on sleep,duration,influence on life and work,influence on emotion and subjective experience(P<0.05),and the scores were lower in the observation group than in the control group(P<0.05).After treatment,the tinnitus loudness score and THI score decreased in both groups(P<0.05),and the intra-group and between-group comparisons showed significant differences at different time points(P<0.05).The total effective rate was 95.5%in the observation group,significantly higher than 80.0%in the control group(P<0.05).Conclusion Acupuncture at Shuaigu(GB8)plus acupoint injection can produce significant efficacy in treating intractable tinnitus.It can effectively mitigate the main symptoms and signs of tinnitus,decrease tinnitus loudness and improve tinnitus handicap degree.
