重组人干扰素α2b喷雾剂和利巴韦林喷雾剂治疗小儿手足口病的效果比较

摘　　要：目的:比较重组人干扰素α2b喷雾剂和利巴韦林喷雾剂在小儿手足口病(HFMD)中的疗效。方法:将确诊的88例HFMD患儿分为干扰素组(n=44)和利巴韦林组(n=44),分别予以重组人干扰素α2b喷雾剂和利巴韦林喷雾剂治疗,所有HFMD患儿均接受常规隔离、清淡饮食、抗病毒、口腔皮肤护理等HFMD的标准干预,持续治疗5 d。观察两组HFMD患儿临床症状缓解情况、实验室指标(WBC、ALT、AST、CK-MB)变化,比较两组临床疗效和不良反应情况。结果:干扰素组退热时间、口腔溃疡消退时间、手足皮疹结痂时间、厌食改善时间均短于利巴韦林组,差异有统计学意义(t=5.924、2.533、2.783、3.478,P<0.05);治疗后,两组WBC、ALT、AST、CK-MB水平均较治疗前明显下降(P<0.05),且干扰素组低于利巴韦林组,差异有统计学意义(t=5.692、3.619、5.193、8.356,P<0.05);两组均未出现明显不良反应,干扰素组总有效率(97.73%)高于利巴韦林组(84.09%),差异有统计学意义(χ^2=4.950,P<0.05)。结论:重组人干扰素α2b喷雾剂缓能有效缓解HFMD患儿临床症状,改善肝功能和减轻心肌损害,临床疗效优于利巴韦林喷雾剂。Objective:To compare the efficacy of recombinant human interferonα2b spray and ribavirin spray in children with hand-foot-mouth disease(HFMD).Methods:88 children with HFMD were randomly divided into the interferon group(n=44)and the ribavirin group(n=44).They were treated with recombinant human interferonα2b spray and ribavirin spray respectively.All HFMD patients received standard intervention of HFMD,such as routine isolation,light diet,antiviral,oral and skin care,and continued treatment for 5 days.The clinical symptoms and laboratory indicators(WBC,ALT,AST,CK-MB)of HFMD children in the two groups were observed,and the clinical efficacy and adverse reactions of the two groups were compared.Results:The time of fever abatement,ulcer subsidence,scab formation and anorexia improvement in interferon group were shorter than those in ribavirin group,the differences were statistically significant(t=5.924,2.533,2.783,3.478,P<0.05).After treatment,the levels of WBC,ALT,AST and CK-MB in both groups decreased significantly(P<0.05),and the above indexes in the interferon group were significantly lower than those in the ribavirin group(t=5.692,3.619,5.193,8.356,P<0.05).There were no significant adverse reactions in both groups.The total effective rate of the interferon group was 97.73%,which was higher than that of the ribavirin group 84.09%(χ^2=4.950,P<0.05).Conclusion:Recombinant human interferonα2b spray can effectively alleviate clinical symptoms,improve liver function and reduce myocardial damage in children with HFMD.The clinical efficacy of recombinant human interferonα2b spray is better than ribavirin spray.
