升高的血压对动脉弹性变化的影响

摘　　要：目的探究30~50岁中青年受检者在升高的血压状态下动脉弹性的变化。方法分别检测正常血压组203例和升高的血压组283例肱踝脉搏波传导速度(baPWV)、踝臂指数(ABI)、颈动脉内-中膜厚度(IMT),并进行对比分析。结果升高的血压组收缩压(SBP)及舒张压(DBP)均明显高于正常血压组(122.93±3.36 vs.109.30±7.37;73.46±4.76 vs.67.90±6.38;P<0.05)。动脉硬化检测指标中升高的血压组baPWV较正常血压组明显增快(1283.02±174.82 vs.1243.69±128.04,P<0.05),IMT比正常血压组明显增厚(0.93±0.24 vs.0.81±0.16,P<0.05),ABI较正常血压组明显降低(1.11±0.09 vs.1.14±0.07,P<0.05)。结论升高的血压对全身动脉的弹性及管壁结构已经产生了不利的影响,提示高血压及其并发症的防治不应局限于高血压患者。Objective:To investigate the changes of arterial elasticity in middle-aged and young subjects aged 30-50 years under elevated blood pressure(EBP).Methods:203 patients in the normal blood pressure(NBP)group and 283 patients in the EBP group were tested for brachial-ankle pulse wave velocity(baPWV),ankle brachial index(ABI)and carotid intima-media thickness(IMT).Results:In the EBP group,systolic blood pressure(SBP)and diastolic blood pressure(DBP)were significantly higher than those in the NBP group(122.93±3.36 vs.109.30±7.37;73.46±4.76 vs.67.90±6.38,P<0.05).The baPWV in the EBP group was significantly faster than that in the NBP group(1283.02±174.82 vs.1243.69±128.04,P<0.05),the IMT was significantly thicker than that in the NBP group(0.93±0.24 vs.0.81±0.16,P<0.05),and ABI was significantly lower than that in the NBP group(1.11±0.09vs.1.14±0.07,P<0.05).Conclusion:Elevated blood pressure has adversely affected the elasticity of the systemic arteries and the wall structure,suggesting that the prevention and treatment of hypertension and its complications should not be limited to patients with hypertension.
