临床药师参与中药注射剂合理使用管控的实践与效果

摘　　要：目的评估临床药师参与的中药注射剂合理使用管控工作的效果。方法临床药师通过制定相关中药注射剂临床使用指导原则、开展合理用药知识培训、参与临床查房进行用药指导、加强处方医嘱点评与问题反馈、参与制定管控制度、将中药注射剂被纳入临床路径等措施,对中药注射剂的临床使用情况进行管控、考核与监督。比较管控前后中药注射剂使用情况、使用强度及使用不合理情况。结果管控后,中药注射剂的使用金额占比、中药注射剂限定日剂量、单药用量排名、不合理用药率均较管控前降低。结论临床药师参与中药注射剂合理使用管控工作可促进中药注射剂的合理用药,成效显著。Objective To evaluate the efficacy of the management and control of rational use of traditional Chinese medicine(TCM)injections participated by clinical pharmacists.Methods Clinical pharmacists conducted an management/control,assessment and supervise for clinical use of TCM injections by several means,including formulating the relevant guiding principles for clinical use of TCM injections,developing training about rational drug use knowledge,participating in clinical rounds for drug use guidance,strengthening comment and feedback on doctor′s advice in the prescriptions,participating in formulating management/control system,and incorporating TCM injections into clinical pathways,etc.The use status,intensity in use and irrational use of TCM injections before and after management/control were compared.Results After control,the proportion of TCM injections consumption amount,defined daily dose of TCM injections,ranking of single drug dosage,and irrational use rate were significantly lower than those before control.Conclusion The participation of clinical pharmacists in the management and control of rational use of TCM injections can promote the rational use of TCM injections,achieving a remarkable efficacy.
