摘 要:目的分析攀枝花市聚集性人感染H7N9禽流感病例的流行病学特征,为人感染H7N9禽流感的处置提供实践经验。方法用现场流行病学方法对病例、密切接触者和可疑暴露环境进行调查分析,用RT-PCR对病例标本和外环境标本进行H7N9禽流感病毒核酸检测。结果病例A于2017-06-07发病,06-18确诊,07-14痊愈出院;病例B于2017-06-13日发病,06-19确诊,06-21死亡,两病例间有流行病学关联,外环境采样检测H7N9禽流感病毒阳性率16.28%。结论病例A有明确禽类接触史,病例B有活禽市场暴露,此为一起聚集性人感染H7N9禽流感疫情,落实活禽市场管控制度对疫情控制有明显效果,活禽跨地区运输是人感染H7N9禽流感疫情扩散的关键。Objective To analyze the epidemiological characteristics of clustered human-infected H7 N9 avian influenza in Panzhihua City, to provide practical experience for prevention and control of human-infected H7 N9 avian influenza. Methods Field epidemiological methods were used to investigate cases, close contacts and suspect exposure environments. Respiratory specimens and environmental specimens of the case were collected, and H7 N9 avian influenza virus nucleic acid was detected by RT-PCR. Results Case A fell ill on June 7, 2017, diagnosed on June 18, and was discharged from hospital on July 14. Case B fell ill on June 13,2017,diagnosed on June19, and died on June 21. There was an epidemiological association between the two cases. the positive rate of H7 N9 avian influenza virus in environmental specimens was 16.28%.Conclusion Case A has a clear history of poultry exposure. Case B is exposed to the live poultry market. This infection case is a cluster of human infection with H7 N9 avian influenza epidemic. The implementation of live poultry market control has obvious effects on epidemic control. The cross-regional transportation of live poultry is the key to the spread of human infection with the H7 N9 avian flu epidemic.
