摘　　要：目的:探究基层医院临床药师以药物不良反应为切入点开展药学服务的内容与方法。方法:通过对克林霉素注射液不良反应案例的调查和分析,提出了解决办法和干预措施。结果:通过相关干预措施的实施,克林霉素注射液不良反应发生明显减少,且未发现严重案例。结论:以不良反应监测为切入点,深入临床发挥临床药师药学专业特长;积极与医师、护士、患者沟通交流并解决实际问题,促进了合理用药;发挥了基层医院临床药师转变工作模式开展药学服务的有效途径。Objective:To explore the content and methods of pharmaceutical care as the starting point in primary hospitals by monitoring adverse drug reactions.Methods:Based on the investigation and analysis of adverse reactions of clindamycin injection,the solving methods and interventions were put forward.Results:Through the implementation of relevant intervention measures,the adverse drug reactions of clindamycin injection were significantly reduced,and no serious cases were found.Conclusions:Taking the monitoring of adverse drug reactions as the entry point,clinical pharmacists’specialty in pharmacy was brought into full play in clinical practice.Actively communicate with doctors,nurses and patients and solve practical problems,promoting rational drug use.It is an effective way for clinical pharmacists in primary hospitals to change their working mode and carry out pharmaceutical care.
