帆状胎盘终止妊娠方式及其对围产结局的影响

摘　　要：目的探讨帆状胎盘孕妇终止妊娠方式及其对围产结局的影响。方法选取2017年1月~2018年12月我院30例帆状胎盘病例作为观察组,并随机选取同期30例健康孕妇作为对照组。比较两组间孕妇分娩方式(剖宫产、阴道分娩、产钳助产)以及围产儿结局(胎儿畸形、新生儿窒息、死胎、新生儿死亡、早产、低体重儿)。结果观察组孕妇剖宫产率为53.3%,明显高于对照组的30%,二者具有差异统计学意义(P <0.05);观察组孕妇阴道分娩率为40%,低于对照组的66.7%,二者具有差异统计学意义(P <0.05);观察组新生儿窒息、胎儿窘迫、早产、低体重儿发生率均高于对照组,二者具有差异统计学意义(P<0.05)。结论帆状胎盘可导致不良围产结局的发生,应当尽早确诊,选择合适的终止妊娠方式。Objective To explore the mode of termination of pregnancy in pregnant women with velamentous cord insertion and its influence on perinatal outcome.Methods 30 cases with velamentous cord insertion in our hospital from January 2017 to December 2018 were selected as observation group, and 30 healthy pregnant women in the same period were randomly selected as control group.The mode of delivery(cesarean section, vaginal delivery, forceps delivery) and perinatal outcomes(fetal malformation, neonatal asphyxia, stillbirth, neonatal death, premature delivery, low birth weight) were compared between the two groups.Results The cesarean section rate of the pregnant women in the observation group was 53.3%, which was significantly higher than that of the control group(30%). There was significant difference between the two groups(P < 0.05).The rate of vaginal delivery in the observation group was 40%, which was significantly lower than 66.7% in the control group(P < 0.05).The incidence of neonatal asphyxia, fetal distress, premature delivery and low birth weight infants in the observation group were higher than those in the control group.There was significant difference between the two groups(P < 0.05).Conclusion Velamentous cord insertion can lead to adverse perinatal outcomes. Early diagnosis should be made and appropriate termination methods should be selected.
