摘 要:目的:探讨颅脑磁共振波谱(MRS)成像技术,分析影响颅脑MRS成像质量的因素,并提出质量控制措施,以提高颅脑MRS成像质量。方法:选取在医院就诊的124例颅脑MRS检查的患者资料,所有患者均为颅脑占位性病变,检查资料齐全且经病理证实。MRS扫描采用多体素3D化学位移成像技术。扫描结束后使用波谱专用后处理软件对MRS进行规范化后处理,观察MRS波谱质量,分析检查失败及成像质量不满足诊断要求的原因,提出质量控制措施。结果:在124例患者中有109例患者颅脑MRS检查配合良好,定位准确,参数设置合理,后处理过程规范,所得谱线基线平稳,化合物位置准确。有15例患者MRS扫描失败或不满足诊断要求,其中1例因抑脂、抑水效果不佳导致谱线杂乱且噪声大;4例因患者配合欠佳导致谱线形态差;10例因匀场效果不佳导致谱线杂乱。在总结失败及波谱成像质量不满足诊断要求后提出了颅脑MRS检查的质量控制措施。结论:颅脑MRS检查前应准备充分,而患者的配合、定位规范准确、参数设置合理以及主磁场均匀度良好是保证颅脑MRS扫描成功及结果可靠的关键,对于提高颅脑MRS检查的质量具有重要意义。Objective:To investigate the imaging technique of brain magnetic resonance spectroscopy(MRS)and analyze the factors that affected imaging quality of brain MRS,and put forward the measures of quality control(QC)so as to improve the imaging quality of MRS.Methods:The data of 124 patients who underwent brain MRS examination were selected.And the lesions of all patients were space-occupying lesion of brain.And their examination data were completed and they had been verified by pathology.MRS scan adopted the imaging technique of multi-voxel three dimensional(3 D)chemical shift.After scan,the post-processing software of special spectrum was used to implement normalized postprocessing for MRS,and observe the quality of MRS,and analyze the reasons that caused the failure of examination and that the imaging quality couldn’t meet the requirement of diagnosis.And then the measures of quality control were proposed.Results:109 cases of 124 patients who underwent brain MRS were well,and their locations were accurate,and their parameters were reasonably,and their post-processing were standard,and the baselines of obtained spectral lines were stable,and the position of compound were accurate.The scans of brain MRS of 15 cases were failure or couldn’t meet diagnostic requirements.In these cases,1 case was disorder in spectral line and had larger noise which reasons were poor effects of restrict-fat and restrict-water,and the morphology of spectral lines of 4 cases were bad because of the poor coordination of them,and the spectral lines of 10 cases were disorder because the effects of shimming were not poor.After summarizing the reason of the failure and that the quality of spectrum imaging couldn’t meet the diagnostic requirement,the QC measures of brain MRS examination were proposed.Conclusion:The preparations pre brain MRS examination should be well,and the coordination of patients,accurate location standards,reasonable parameter setting and well evenness of main field were the keys that ensure the success and reliable re
