乳腺青春期巨大纤维腺瘤全数字化X射线与磁共振成像影像表现研究

摘  要:目的:分析乳腺青春期巨大纤维腺瘤(JGF)全视野数字化乳腺X射线摄影(FFDM)与磁共振成像(MRI)影像表现,并与手术后病理对照分析,以期提高其诊断水平。方法:收集医院收治的经乳腺FFDM检查并手术病理证实的7例(9个病灶)JGF患者影像资料,其中3例患者行MRI平扫、弥散加权成像(DWI)及6期动态对比增强扫描。分析其临床表现、FFDM和MRI影像学表现。结果:FFDM显示为乳内单发或多发圆形或椭圆形巨大肿块,病灶最大径7.013.0 cm,平均约9.5 cm,所有肿块密度均匀,无钙化,其中5个肿块边缘见透亮晕征,4个肿块因被致密腺体遮蔽而边界不清,肿块大小与触诊大小相仿。MRI显示为3个病灶均表现为边界清楚巨大肿块,T1WI呈等低信号,T2WI以中高信号为主,内见斑片状更高信号变性坏死区和低信号流空血管影,病灶内分隔与周围包膜T2WI均呈低信号,DWI病灶呈高信号,平均表观弥散系数(ADC)值为1.44×10^-3mm^2/s;动态增强病灶实性成分呈不均匀渐进性明显强化,病灶内分隔与包膜无强化呈低信号,3例时间-信号强度曲线(TIC)均为流入型,增强峰值高。结论:JGF具有一定临床X射线及MRI影像学特征性表现,其主要表现为青春期发病,病灶一般较大且生长快;FFDM及MRI均表现为良性肿块特点,故X射线与MRI结合,可以提高其诊断与鉴别水平。Objective:To analyze imaging manifestation of full field digital mammography(FFDM)and magnetic resonance imaging(MRI)for juvenile giant fibroadenoma(JGF)of the breast,and to implement contrast analysis with postoperatively pathological results so as to improve diagnostic level.Methods:The imaging data of 7 patients with 9 lesions which were confirmed by FFDM examination on breast and surgical pathology were collected.Three patients of them underwent MRI plain scan,diffusion weighted imaging(DWI)and dynamic contrast-enhanced scan at 6 phases.And the clinical manifestations of these patients and the image manifestations of FFDM and MRI of them were analyzed.Results:The results of FFDM showed single or multiple round or oval giant masses in the breast,and the maximum diameter of these lesions was 7.0-13.0 cm,and the mean of them was 9.5 cm,and the densities of all lumps were uniform and there was no calcification in these lumps.Clear halo sign could be seen on the edge of 5 lumps,and there were obscure boundary on 4 lumps because they were obscured by dense glands and the size of these lumps was similar to that of palpation for them.The results of MRI indicated 3 lesions appeared giant lumps with clear boundary,and T1 WI of them appeared iso-low signal and T2 WI of them mainly appeared middle-high signal.And the higher signal intensity denatured necrotic region with plaque and flow empty vascular shadow of low signal were found in the interior of lesions.The T2 WI both internal septum of lesions and periphery envelope appeared low signals,and the DWI of lesion appeared high signals,and the mean of ADC value was 1.44×10^-3 mm^2/s.The dynamic enhancement of the solid components of the lesions appeared non-uniform,progressive and obvious enhancement.And the signal intensity of the internal septum and the envelope in lesions appeared low signal.The time-signal intensity curves(TIC)of 3 cases were inflow type,and the peak value of enhancement was high.Conclusion:JGF has a certain manifestation of imaging characteristics
