摘 要:目的探讨五酯胶囊对他克莫司在肝移植患者体内药代动力学方面的影响。方法选取2013年1月至2014年8月在海军军医大学附属长征医院行同种异体尸体肝移植的15例患者为研究对象,其中:男性11例,女性4例,年龄(51.72±8.08)岁。分别在患者单独服用他克莫司,以及服用他克莫司一段时间后加服五酯胶囊,即同时服用他克莫司与五酯胶囊时,采集血样。血样采集时间为服药后即刻、0.5 h、1 h、2 h、4 h、8 h、12 h、16 h、20 h、24 h;分别计算2种情况下患者的药代动力学参数:达峰时间(Tmax)、分布容积(V/F)和CLz/F。结果与单独服用他克莫司时相比,同时服用他克莫司与五酯胶囊时,他克莫司的Tmax明显缩短(3.2 vs.2.0,P<0.05)、V/F显著降低(388.969 vs.135.632,P<0.05),且CLz/F明显下降(23.935 vs.13.867,P<0.05)。结论五酯胶囊对肝移植患者体内他克莫司代谢为有益影响。Objective To investigate the effects of Wuzhi capsules on the pharmacokinetics of tacrolimus in patients with liver transplantation.Methods Fifteen patients 11 were male and 4 were female with first allogeneic corpse liver transplantation in Shanghai Changzheng Hospital from January 2013 to August 2014 were selected as study subjects.Blood samples were collected respectively when patients were taking tacrolimus alone,and when they were taking tacrolimus for a period of time and then supplemented with tacrolimus and Wuzhi capsules.The time points for taking blood samples were:the time immediately after taking the drug,0.5 h,1 h,2 h,4 h,8 h,12 h,16 h,20 h and 24 h after medication.The pharmacokinetic parameters of the patients under the following conditions were calculated:the time to peak(Tmax),the volume of distribution(V/F)and CLz/F.Results The Tmax of tacrolimus significantly shortened for those patients simultaneously taking tacrolimus and Wuzhi capsules,as compared with those who simply took tacrolimus(3.2 vs.2.0)(P<0.05),while V/F(388.969 vs 135.632)and CLz/F(23.935 vs.13.867)significantly reduced(P<0.05).Conclusion Wuzhi capsules could produce beneficial effects on the metabolism of tacrolimus in the patients with liver transplantation.
