摘 要:目的探讨超声、 CT和MRI诊断肾上腺占位病变的临床价值。方法选取2017年12月至2019年12月我院收治的疑似肾上腺占位病变患者98例,术前予以超声、 CT和MRI检查。以病理检查结果为金标准,比较三种检查方式的诊断效能。结果超声阳性检出率为76.53%(75例),CT阳性检出率为78.57%(77例),MRI阳性检出率为89.80%(88例),病理检查阳性检出率为91.84%(90例)。超声和CT阳性检出率比较差异无统计学意义(P>0.05)。MRI和病理检查阳性检出率均明显高于超声和CT (P <0.05)。MRI的敏感性、阳性预测值、阴性预测值均明显高于超声(P <0.05)。CT与超声的特异性、敏感性、阳性预测值和阴性预测值比较差异无统计学意义(P>0.05)。CT与MRI的特异性和阳性预测值比较差异无统计学意义(P>0.05)。MRI的敏感性和阴性预测值均明显高于CT (P <0.05)。结论超声、 CT、 MRI在肾上腺占位病变中均有较高的诊断价值,其中MRI的特异性和敏感性最高,应用价值最显著,但在临床检验过程中仍需参考其他的检查方式,综合考虑检查结果以此提高诊断的准确率。Objective To explore the clinical value of ultrasound, CT and MRI in the diagnosis of adrenal mass. Methods 98 patients with suspected adrenal mass admitted to our hospital from December 2017 to December 2019 were selected and examined by ultrasound, CT and MRI before surgery. Taking pathological examination results as the gold standard, the diagnostic efficacy of three examination mothods were compared. Results The positive detection rate of ultrasound was 76.53%(75 cases). The positive detection rate of CT was 78.57%(77 cases). The positive detection rate of MRI was 89.80%(88 cases). The positive detection rate of pathological examination was 91.84%(90 cases). No statistical difference was found in the positive detection rate between ultrasound and CT(P >0.05). The positive detection rates of MRI and pathological examination were significantly higher than those of ultrasound and CT(P <0.05). The sensitivity, positive predictive value and negative predictive value of MRI were significantly higher than those of ultrasound(P <0.05). No statistical difference was found in the specificity, sensitivity, positive predictive value and negative predictive value between CT and ultrasound(P >0.05). No statistical difference was found in the specificity and positive predictive value between CT and MRI(P >0.05). The sensitivity and negative predictive value of MRI were significantly higher than those of CT(P <0.05). Conclusions Ultrasound, CT and MRI have high diagnostic value in adrenal mass. Among them, MRI has the highest specificity and sensitivity, and the most significant application value. However, during the process of clinical test, it is still necessary to refer to other examination methods and comprehensively consider the examination results to improve the diagnostic accuracy.
