摘　　要：目的回顾性分析不同预后的桥本甲状腺炎(HT)合并甲减患者的临床特征。方法根据观察终点血过氧化物酶抗体(TPOAb)是否恢复正常,将104例HT合并甲减患者分为缓解组(n=54)及未缓解组(n=50)。比较两组的基线资料及治疗前后甲状腺功能指标、体重、 L-T4剂量和尿碘中位数(MUI)。结果两组的BMI、性别比例、年龄、血压、心率、 L-T4时间比较无统计学差异(P>0.05);未缓解组男性吸烟比例明显高于缓解组(P <0.05)。在观察终点两组的甲状腺功能恢复正常;缓解组的TGAb、 TPOAb、体重、 MUI及L-T4剂量明显低于未缓解组(P <0.01或P <0.05)。结论控烟、控制体重、减少碘摄入可能提高HT合并甲减患者的缓解率,改善预后。Objective The clinical characteristics of Hashimoto’s thyroiditis(HT) complicated with hypothyroidism in patients with different prognosis were retrospectively analyzed. Methods According to whether the blood peroxidase antibody(TPOAb) at endpoint falling into normal range, 104 patients with HT complicated with hypothyroidism were divided into remission group(n = 54) and non-remission group(n = 50). The baseline data, and the thyroid function indicators, body weight, L-T4 dose and median urine iodine(MUI)before and after treatment were compared between the two groups. Results No statistical difference was found in the BMI, sex ratio, age,blood pressure, heart rate and L-T4 time between two groups(P >0.05). The male smoking rate of the non-remission group was higher than that of the remission group(P <0.05). At the observation endpoint, the thyroid function returned to normal levels in both groups;The TGAb,TPOAb, body weight, MUI and L-T4 dose of the remission group were significantly lower than those of non-remission group(P <0.01 or P<0.05). Conclusions Tobacco control, weight control and limiting iodine intake may improve the remission rate and prognosis in patients with HT complicated with hypothyroidism.
