摘  要:目的探索运动结合饮食干预对HDL亚类分布的影响。方法从皓千减肥夏令营11-17岁男性营员中选取肥胖青少年12名为研究对象,以同龄体重正常、男性青少年6名为对照。进行为期4周以饮食控制和运动干预为主的封闭式减肥夏令营。于入营第一天和第四周末,采集空腹肘静脉血5mL,进行血清HDL亚类preβ1-HDL、preβ2-HDL、preβ3-HDL、HDL3a、HDL3b、HDL3c、HDL2a及HDL2b的测试,以分析肥胖青少年血清HDL亚类的分布特点及运动结合饮食干预对HDL亚类分布的影响。结果(1)肥胖青少年血清HDL亚类中以preβ-HDL为主,占比90%以上,HDL3和HDL2所占较小。(2)与体重正常组比较,肥胖组血清总HDL水平无差异(P>0.05),但是HDL亚类中HDL2a(P<0.001)和HDL2b(P<0.01)含量显著升高,其他亚类无显著性差异(P>0.05)。(3)4周运动结合饮食干预未改变肥胖青少年血清中HDL总体水平(P>0.05),但是亚类中HDL2(P<0.05)和HDL3(P<0.05)含量下降,preβ-HDL含量(P<0.001)显著上升。结论肥胖青少年血清HDL亚类中以preβ-HDL为主。但是HDL2a和HDL2b含量高于体重正常青少年。4周的运动结合饮食干预可显著降低HDL2和HDL3含量,升高preβ-HDL含量,即改变HDL的亚类分布,但不改变HDL总水平。The purpose of this study was to explore the effect of exercise combined with diet intervention on the distribution of HDL subclass.12 obese adolescents were selected from male campers aged 11-17 in Haoqian Summer Camp,with 6 normal-weight adolescents taken as controls.Exercise intervention combined with diet control was conducted for 4 weeks.Fasting elbow venous blood 5 ml was collected on the first day and the last day.Serum HDL subclasses preβ1-HDL,preβ2-HDL,preβ3-HDL,HDL3a,HDL3b,HDL3c,HDL2a and HDL2b were analyzed by ELISA.The results showed that:(1)Preβ-HDL was the main subclass in obese adolescents,accounting for more than 90%,while HDL3 and HDL2 accounted for less.(2)Compared with the normal group,the serum total HDL level in obese group had no difference(P>0.05),but HDL2a(P<0.001)and HDL2b(P<0.01)significantly increased(P>0.05).(3)The four-week exercise combined with diet intervention did not change the overall level of HDL(P>0.05),but HDL2(P<0.05)and HDL3(P<0.05)was decreased,while preβ-HDL(P<0.001)was increased significantly.It could be concluded that Preβ-HDL was the main subtype in obese adolescents.However,HDL2a and HDL2b were higher than those of normal weight adolescents.Four weeks of exercise combined with diet intervention could significantly reduce HDL2 and HDL3,increase preβ-HDL,which meant that there was a change in the distribution of HDL subclasses,but not in the total level of HDL.
