摘 要:目的研究脑电图监测在儿童重症颅内感染诊断中的应用价值。方法回顾性分析2015年6月-2018年6月郑州大学附属儿童医院收治的71例重症颅内感染患儿的临床资料,所有患者均进行脑电图监测,分析脑电图监测应用于重症颅内感染患儿的诊断价值。结果不同病因的重症颅内感染患儿脑电图异常表现分布显著不同,说明颅内感染病因能够较大程度影响脑电图表现。入院初次阳性检出67例(94.37%);经治疗,1周后复查48例,阳性检出35例(72.92%);2周后复查23例,阳性检出19例(82.61%);1个月后复查5例,阳性检出5例(100.00%)。首检慢波,复查显示电静息患儿病情加重率较高;首检痫样放电,复查显示放电导联范围缩小及复查无显著变化患儿加重率次之;首检慢波,复查波幅增高或频率加快及首检痫样放电,复查显示慢波患儿无加重情况发生。异常组蛋白质定量及白细胞计数水平低于健康组。结论在应用于儿童重症颅内感染时,脑电图诊断阳性检出率较高,对儿童重症颅内感染诊断及临床治疗具有积极影响,此外,脑电图还能够为病情发展预估提供科学依据,以此做出预防性治疗措施,降低患儿病痛,具有较高临床推广价值。Objective To study the value of electroencephalogram(EEG)monitoring in the diagnosis of severe intracranial infection in children.Methods The clinical data of 71 children with severe intracranial infection from June, 2015 to June, 2018 were analyzed retrospectively. All patients were monitored by electroencephalogram(EEG). The diagnostic value of EEG monitoring in children with severe intracranial infection was analyzed.Results The abnormal distribution of electroencephalogram(EEG) in children with severe intracranial infection with different causes was significantly different,which indicated that the etiology of intracranial infection could affect the electroencephalogram(EEG)manifestations to a great extent. 67 cases(94.37%)were positive for the first time after admission,and 48 cases were reexamined one week after treatment. 35 cases(72.92%) were positive. After 2 weeks,23 cases were rechecked,and 19 cases were positive(82.61%). 5 cases were examined after 1 month,and 5 cases were positive(100.00%). The first examination of slow wave showed that the rate of exacerbation was higher in children with electrostatic rest. The first examination of epilepsy discharge showed that the discharge lead range was reduced,and the rate of exacerbation in children with no significant change in the reexamination was the second. The first slow wave was detected,the wave amplitude was increased or the frequency was increased,and the first-stage epilepsy was discharged,and the review showed that there was no exacerbation in children with slow wave.Conclusion In children with severe intracranial infection, electroencephalogram has higher positive detection rate and severe cranium. The diagnosis and clinical treatment of internal infection have a positive impact. In addition,electroencephalogram(EEG)can also provide scientific basis for the development of the disease,so as to make preventive treatment measures and reduce the pain of children. It has a high clinical promotion value.
