摘  要:目的探讨核磁共振成像(MRI)联合三维时间飞跃法磁共振血管成像技术(3D-TOF MRA)对急性基底动脉尖综合征的诊断价值。方法选取郑州市第一人民医院2018年3月-2019年6月期间收治的95例疑似急性基底动脉尖综合征患者,均给予患者MRI联合3D-TOF MRA检查,以数字减影血管造影技术(DSA)检查结果作为金标准,分析DSA、MRI联合3D-TOF MRA检查的影像学表现及MRI联合3D-TOF MRA检查结果。结果 MRI联合3D-TOFMRA对急性基底动脉尖综合征的诊断准确率为95.79%,敏感度为96.30%,特异度为92.86%;经MRI联合3D-TOFMRA检查78例急性基底动脉尖综合征患者的病变部位如下,丘脑及中脑处发生病变的患者最多,各为19例,占比为24.36%,其次为枕叶处,有11例,占比为14.10%,侧脑室旁发生病变的患者最少,有4例,占比为5.13%。结论使用MRI联合3D-TOF MRA对急性基底动脉尖综合征患者的诊断准确率较高,且可明确病变部位,可在临床上推广使用。Objective To evaluate the diagnostic value of magnetic resonance imaging(MRI)combined with three-dimensional time-leap-method magnetic resonance angiography(3D-TOF MRA)on acute basilar artery-tip syndrome.Methods 95 patients with suspected acute basilar artery-tip syndrome treated in the hospital from March, 2018 to June, 2019 were given MRI combined with 3D-TOF MRA, and the DSA was analyzed by digital subtraction angiography(DSA). MRI combined with 3D-TOF MRA for imaging and MRI in 3D-TOF MRA results of 3D-TOF MRA examination were analyzed.Results The diagnostic accuracy,sensitivity and specificity of MRI combined with 3D-TOF MRA in acute basilar artery syndrome were 95.79%,96.30% and92.86% respectively. The lesion sites of 78 patients with acute basilar artery cusp syndrome examined by MRI and 3D-TOF MRA were as follows,19 patients(24.36%)had lesions in thalamus and midbrain,11 patients(14.10%)had lesions in occipital lobe,and 4 patients(5.13%) had paraventrallesions in lateral ventricle.Conclusion The diagnostic accuracy of MRI combined with3D-TOF MRA in patients with acute basilar artery apex syndrome is high, and the location of the lesion can be popularized in clinic.
