快速康复外科护理对髋关节置换术后患者关节功能及生活质量的影响

摘　　要：目的探讨基于快速康复外科(ERAS)理念的护理对髋关节置换术后患者关节功能及生活质量的影响。方法选取2015年1月—2018年12月在四川省遂宁市中医院就诊的350例髋关节置换术后患者,随机分为常规组和ERAS组,每组各175例。常规组给予常规护理,ERAS组给予基于ERAS的护理干预。比较两组的拔管时间、下床活动时间、住院时间、Harris评分、生活质量(SF-36)评分及住院医疗费用。结果ERAS组患者拔管时间(30.2±10.4)min、首次下床活动时间(1.2±0.6)d、住院时间(10.7±3.2)d,均短于常规组,差异有统计学意义(t值分别为12.975、14.349、9.586;P<0.01)。ERAS组患者Harris总评分(87.2±5.5)分,常规组(81.6±6.3)分,两组比较差异有统计学意义(t=8.858,P<0.01)。ERAS组患者SF-36量表总体健康维度评分(81.35±5.81)分,常规组(74.17±5.16)分,两组比较差异有统计学意义(t=12.223,P<0.01)。ERAS组的化验费、床位费、护理费、药费、总住院费用低于常规组,差异有统计学意义(P<0.01)。结论基于ERAS的护理干预对髋关节置换术后患者的康复具有较好的临床效果,可改善患者髋关节功能,促进康复进程,提高生活质量。Objective To explore the effects of nursing based on the concept of enhanced recovery after surgery(ERAS)on joint function and quality of life of patients after hip replacement.Methods A total of 350 patients after hip replacement who were treated in Suining Municipal Hospital of TCM from January 2015 to December 2018 were selected and they were randomly divided into the conventional group and the ERAS group,with 175 cases in each group.The conventional group was given conventional nursing intervention while the ERAS group was given ERAS-based nursing intervention.The extubation time,ambulation time,hospital stay,Harris score,quality of life(SF-36)score and hospitalization medical expenses in the two groups were compared.Results The extubation time,ambulation time and hospital stay in the ERAS group were respectively(30.2±10.4)min,(1.2±0.6)d and(10.7±3.2)d,all significantly shorter than those in the conventional group,the differences were statistically significant(t=12.975,14.349,9.586;P<0.01).The Harris score of patients in the ERAS group was(87.2±5.5)points and that in the conventional group was(81.6±6.3)points,the difference between the two groups was statistically significant(t=8.858,P<0.01).The general health of SF-36 scale of patients in the ERAS group in the ERAS group was(81.35±5.81)points and that in the conventional group was(74.17±5.16)points,the difference between the two groups was statistically significant(t=12.223,P<0.01).The laboratory fees,bed fees,nursing expenses,medicine expenses and total hospitalization expenses of the ERAS group were all lower than those in the conventional group,the differences were statistically significant(P<0.01).Conclusions ERAS-based nursing intervention has a good clinical effect on the rehabilitation of patients after hip replacement.It can significantly improve hip function,promote the rehabilitation process and improve the quality of life.
