摘  要:目的探讨血管紧张素原(AGT)基因多态性与广州市居民原发性高血压(EH)的相关性及EH的危险因素。方法从广州市户籍居民中选取537名已确诊的EH患者及同期年龄、性别、居住区域相匹配的545名血压正常居民为研究对象,应用MassARRAY~?IPLEX SNP分型技术对AGT基因3个多态性位点rs5050、rs5051和rs699进行检测,并结合研究对象的一般资料、生活方式、患病情况、体格检查及实验室检测结果进行统计学分析。结果rs5051和rs699两个位点的基因型分布在EH组和对照组差异具有统计学意义(P<0.05),多因素Logistic回归分析显示,吸烟、超重、肥胖、中心性肥胖、糖尿病、高低密度脂蛋白胆固醇、rs5051 CC基因型及rs699 GG基因型均为EH的危险因素(P<0.05),而高高密度脂蛋白胆固醇为EH的保护因素(OR=0.651,95%CI:0.455~0.931,P=0.019)。结论AGT基因多态性与广州市居民EH发病风险相关,应综合考虑遗传和环境因素及早进行EH防控。Objective To investigate the association of angiotensinogen gene polymorphisms with essential hypertension(EH)and related risk factors among Guangzhou residents.Methods A total of 537 EH patients and 545 healthy controls matched with age,gender and residential areas were enrolled.Three SNPs of AGT,rs5050,rs5051 and rs699,were genotyped via MassARRAY?IPLEX SNP platform.Statistical analysis was performed according to the genotypes,general information,lifestyle,illness,physical and laboratory test results of the subjects.Results The distribution of genotype for rs5051 and rs699 was significantly different between EH and control group(P<0.05).Multivariate logistic regression analysis showed that smoking,overweight,obesity,central obesity,diabetes,high LDL-C,rs5051-CC and rs699-GG revealed higher risk of EH(P<0.05),however,high HDL-C could decrease the risk of EH(OR=0.651,95%CI:0.455-0.931,P=0.019).Conclusion Polymorphisms of AGT gene were associated with EH among Guangzhou residents and both genetic and environmental factors should be taken into account to prevent EH in early stage.
