摘  要:目的通过对2018年乌鲁木齐市手足口病的检测数据的分析,掌握手足口病病毒的组成特性以及病原变化情况,为手足口病防控策略的制定提供依据。方法从"传染病信息管理监测系统"中导出2018年乌鲁木齐市手足口肠道病毒的监测数据,采用c2检验以及Fisher确切概率法计算概率分析手足口肠道病毒的流行病学特征。结果 1 569份标本中检出肠道病毒阳性1 370份,其中比例最少的是肠道病毒71型(EV71)为1.17%,柯萨奇病毒A16型(CA16)为26.06%,而最多的是柯萨奇病毒A6型(CA6)比例高达72.77%。由于2018年乌鲁木齐市以CA6为主要病原,且阳性检出率偏高,一年四季不同区域、年龄、性别、职业间的阳性检出率在各个分段之间相差不大,标本的阳性检出率只在不同季度中的差异有统计学意义(P<0.05)。病原分型在不同性别、职业等肠道病毒病原构成的差异均无统计学意义(P>0.05),而在不同区域、季度的差异均具有统计学意义(P<0.05)。结论 2018年乌鲁木齐市手足口病的病原中CA6构成比过高,成为当年的优势病原。目前并没有针对CA6型的疫苗,加强手足口肠道病毒的监测,及时掌握疾病流行特征,对合理配置卫生资源,科学制定预防与治疗方案具有重要意义。Objective By analyzing the detection data of hand,foot and mouth disease(HFMD)in Urumqi in 2018,to grasp the composition and characteristics of hand,foot and mouth disease virus and the change of pathogen,and to provide the basis for formulating the prevention and control strategy of hand,foot and mouth disease. Methods The surveillance data of hand-foot-mouth enterovirus in Urumqi in 2018 were obtained from the"Infectious Disease Information Management and Monitoring System". Epidemiological methods were used to analyze the characteristics of hand-foot-mouth enterovirus.Results 1 370 of 1 569 specimens were positive for enterovirus. Enterovirus 71(EV71),coxsackievirus A16(CA16)and coxsackievirus A6(CA6)accounted for 1.17%,26.06% and 72.77%,respectively. CA6 was the main pathogen in 2018,and the detection rate was high. Except for the difference of positive detection rate in different months,the positive detection rate in other regions,age,sex and occupation was more than 80%,and the difference was not significant. The positive detection rate in different months and regions had statistical significance(all P<0.05). There was no significant difference in the composition of enterovirus pathogens in different regions,time,sex,age and occupation(P>0.05). Conclusions CA6 was the dominant pathogen of HFMD in Urumqi in 2018. It is very important to strengthen the etiological surveillance of HFMD and master the pathogenic characteristics of HFMD and the genetic variation of important pathogens in the prevention and control of HFMD.
