烟台市1954例HIV抗体初筛阳性血清复检及确证结果分析

摘　　要：目的分析烟台市1 954份艾滋病病毒(HIV)抗体复检与确证实验的准确性。方法对2014年1月-2018年12月烟台市艾滋病筛查实验室初筛阳性送检血清1 954份,使用伯乐四代酶联免疫法诊断试剂(ELISA)和雅培硒标快检试剂条(RT)复检,复检阳性可疑血清进行免疫印迹法(WB)确证检测。结果复检样品总数为1 954份,其中复检双阳性1 580份(80.86%),复检单阳性67份(3.42%),复检双阴性307份(15.71%)。复检双阳性样品1 580份,其中确证阳性1 516份,不确定63份,确证阴性1份。复检单阳性样品67份,结果为确证阳性4份,不确定12份,确证阴性51份。2014-2018年HIV不确定率分别为3.50%、2.94%、2.84%、6.04%、8.33%。结论烟台市HIV不确定病例逐年增多,应重视初筛试验假阳性增多的现象,加强随访检测和HIV核酸检测以免误诊。Objective To analyse the accuracy of human immunodeficiency virus(HIV) antibody testing with different tests in Yantai. Methods 1 954 samples which were positive with primary screening test for HIV were reexamined by ELISA kit(BIO-RAD)and a rapid kit(Abbot Determine HIV-1/2). All positive samples were comfirmed by WB test.Results Of 1 954 samples,1 580 were both positive with ELISA and RT. Of 1 580 samples,1 516 were positive with WB,63 were uncertained and 1 was negative. Out of 67 positive samples tested with ELISA or RT,4 were positive with WB,51 were uncertained and 12 were negative.The uncertainty rates from 2014 to 2018 were 3.50%,2.94%,2.84%,6.04% and8.33%,respectively. Conclusion The uncertainty rate was increasing in Yantai from 2014 to 2018 and more attention should be paid to the increase of false positivity in HIV screening laboratories. The Follow-up should be taken and the detection of HIV nucleic acid should be performed.
