童趣化护理干预对学龄前先天性唇腭裂整形手术患儿心理应激反应及术后疼痛程度的影响

摘　　要：目的探讨童趣化护理干预对先天性唇腭裂整形手术患儿心理应激反应及术后疼痛程度的影响。方法选取2018年4月~2020年3月在我院接受唇腭裂整形修复术的54例先天性唇腭裂学龄前患儿为研究对象,采用随机数字表法将其分为观察组和对照组,每组27例。对照组给予常规护理,观察组在常规护理的基础上给予童趣化护理干预。运用东大略儿童医院疼痛评分(CHEOPS)评价干预前后两组患儿疼痛程度;采用汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)评价干预7d前后患儿心理应激反应程度,统计两组患儿住院天数。结果干预前,两组患儿HAMA、HAMD评分组间比较差异均无统计学意义(P> 0.05),干预后,干预后观察组患儿HAMA、HAMD评分明显较干预前下降,且明显低于对照组,观察组干预前后组内比较、干预后组间比较,差异均有统计学意义(P <0.05)。术后观察组CHEOPS评分明显低于对照组,差异有统计学意义(t=5.728,P=0.039)。观察组评价住院天数较对照组短,差异有统计学意义(t=8.967,P=0.011)。结论童趣化护理干预可有效减轻学龄前先天性唇腭裂整形手术患儿心理应激反应,缓解术后疼痛程度,提高患儿治疗依从性和护理配合度,缩短住院天数。Objective To explore the effect of childlike nursing intervention on psychological stress and postoperative pain in children with congenital cleft lip and palate surgery.Methods A total of 54 preschool children with congenital cleft lip and palate who underwent repair of cleft lip and palate in our hospital from April 2018 to March 2020 were selected as research objects.They were divided into observation group and control group by random number table method.Group of 27 patients.The control group was given routine care,and the observation group was given childlike nursing intervention on the basis of routine care.The Dongdaluo Children's Hospital Pain Score(CHEOPS)was used to evaluate the pain of the two groups of children before and after the intervention.The Hamilton Anxiety Scale(HAMA)and the Hamilton Depression Scale(HAMD)were used to evaluate the psychological stress of the children before and after the intervention Degree,count the number of days of hospitalization of children in the two groups.Results Before the intervention,there was no significant difference in the HAMA and HAMD scores between the two groups of children(P>0.05).After the intervention,the HAMA and HAMD scores of the children in the observation group after the intervention were significantly lower than those before the intervention and were significantly lower than the control.There were statistically significant differences between the groups before and after the intervention,and between the groups after the intervention(P<0.05).The cheops score of the observation group was significantly lower than that of the control group after surgery,and the difference was statistically significant(t=5.728,P=0.039).The number of hospital stays in the observation group was shorter than that in the control group,and the difference was statistically significant(t=8.967,P=0.011).Conclusion Childlike nursing intervention can effectively reduce the psychological stress response of children with congenital cleft lip and palate surgery before school age,reliev
