摘  要:乳腺癌是女性中发病率最高、最常见的恶性肿瘤,其10年生存率可达60%,早期乳腺癌得到有效治疗后,存活率可达80%,早期发现并积极进行临床诊治非常重要。本研究分别以动态增强扫描(dynamic contrast enhanced magnetic resonance imaging,DCE-MRI)、磁共振弥散加权成像(diffusion weighted imaging,DWI)、磁共振波谱成像(magnetic resonance spectroscopy imaging,MRS)、T2*加权灌注成像(T2*-weighted perfusion weightedimage,T2*-PWI)和弥散张量成像(diffusion tensor imaging,DTI)等为关键词,检索2015-01-2019-01中国知网、万方及PubMed数据库发表的文献,纳入标准:(1)乳腺良恶性病变诊断;(2)磁共振技术在乳腺病变中应用。排除标准:(1)综述、重复发表的文献;(2)非乳腺病变诊断。最终纳入并分析29篇高度相关文献。DCE-MRI、DWI、MRS、T2*-PWI和DTI从血流动力学、细胞分子学方面不仅可以提高判断病变良恶性的特异度和灵敏度,还可以对术前及预后进行评估。多参数核磁共振对乳腺占位良恶性及术前预后的评估具有重要意义。Breast cancer is the highest incidence and most common malignant tumor in women.Its 10-year survival rate can reach 60%.After effective treatment of early breast cancer,the survival rate can reach 80%.Early detection and active clinical diagnosis and treatment are very important.This study used dynamic contrast enhanced magnetic resonance(DCE-MRI),magnetic resonance diffusion weighted imaging(DWI),magnetic resonance spectroscopy imaging(MRS),T2*-weighted perfusion weighted image(T2*-PWI)and diffusion tensor imaging(DTI)as the keywords were retrieved from January 2015,to January 2019 published by China HowNet,Wanfang and PubMed databases.Literature inclusion criteria:(1)diagnosis of benign and malignant breast lesions;(2)application of magnetic resonance technology in breast lesions.Exclusion criteria:(1)review and republished literature;(2)non-breast disease diagnosis.Finally,29 highly relevant literatures were included and analyzed.DCE-MRI,DWI,MRS,T2*-PWI and DTI can not only improve the specificity and sensitivity of judging the benign and malignant lesions from the aspects of hemodynamics and cell molecular science,but also can evaluate preoperative and prognosis.Multi-parameter MRI is of great significance for the evaluation of benign and malignant breast mass and its prognosis.
