先兆子痫患者胎盘Kiss-1基因表达与生理指标相关性分析

摘　　要：目的先兆子痫是孕妇围产期常见并发症之一,常伴随相关基因表达异常。本研究分析胎盘Kiss-1基因表达水平与先兆子痫相关生理指标的关系,探讨胎盘Kiss-1水平对先兆子痫胎儿发育影响。方法收集2017-07-01-2019-01-01泰安市中心医院妇产科收治的42例先兆子痫患者组织标本,根据平均动脉压和24h尿蛋白水平的不同,分为轻度组和重度组;收集同院同期产科检查的42例正常孕妇组织标本作为对照。采用RT-PCR测定胎盘绒毛膜滋养层细胞Kiss-1 mRNA表达水平,评估其与孕妇平均动脉压、24h尿蛋白及产儿体质量相关性,以及对胎儿股骨长度、双顶径及胎盘厚度影响。结果重度组胎盘Kiss-1 mRNA表达水平为2.37±0.36,高于轻度组的1.86±0.25和对照组的1.16±0.17,组间比较差异有统计学意义,F=5.408,P=0.0103。Kiss-1表达与孕妇平均动脉压呈正相关,r=0.624,P=0.028;与24h尿蛋白呈正相关,r=0.427,P=0.008;与产儿体质量呈负相关,r=-0.728,P=0.001。Kiss-1≥1.5组胎盘厚度(t=2.481,P=0.012)、胎儿股骨长度(t=3.247,P=0.001)及胎儿双顶径(t=2.762,P=0.006)均低于Kiss-1<1.5组。结论先兆子痫患者Kiss-1水平与多个生理指标相关,可以用于监测先兆子痫发生及胎儿发育情况。OBJECTIVE Pre-eclampsia is a common complication of perinatal period in pregnant women,often accompanied by abnormal expression of related genes.This study evaluated the role of placental Kiss-1 in the pathogenesis of pre-eclampsia and explored the relationship between placental Kiss-1 expression levels and physiological indicators associated with pre-eclampsia.METHODS Forty-six patients with preeclampsia admitted to the Department of Obstetrics and Gynecology,Taian Central Hospital from July 1,2017 to January 1,2019 were selected as subjects.According to the mean arterial pressure and 24 hurinary protein levels,they were divided into mild groups and severe group.Totally 42 normal pregnant women who were selected for the same time were selected as the control group.The expression of Kiss-1 mRNA in placental chorionic trophoblast cells was determined by RT-PCR,and its correlation with mean arterial pressure,24 hurine protein and body mass of pregnant women,as well as fetal femur length,double top diameter and placental thickness were evaluated.RESULTS The expression level of Kiss-1 in the severe group was 2.37±0.36,which was higher than 1.86±0.25 in the mild group and 1.16±0.17 in the control group.The difference between the groups was statistically significant,F=5.408,P=0.0103.There was a positive correlation between Kiss-1 and the average arterial pressure of pregnant woman(r=0.624,P=0.028),which was positively correlated with 24-hour urine protein(r=0.427,P=0.008),and negatively correlated with body weight(r=-0.728,P=0.001).Kiss-1≥1.5 group placental thickness(t=2.481,P=0.012),fetal femur length(t=3.247,P=0.001),fetal biparietal diameter(t=2.762,P=0.006)were lower than Kiss-1<1.5 groups.CONCLUSION The level of Kiss-1 in preeclampsia patients is related to many physiological indexes,it can be used to monitor the occurrence of preeclampsia and fetal development.
