近10年文献中推拿手法治疗膝关节骨性关节炎应用规律探讨

摘　　要：目的:探讨推拿治疗膝关节骨性关节炎近10年文献中潜在手法应用规律,为推拿治疗膝关节骨性关节炎的手法辨证施治提供一些参考依据和理论指导。方法:通过统一的检索式分别对万方、维普、CNKI三大中文数据库中推拿治疗膝关节骨性关节炎的相关文献进行检索,合并三大数据库中的重复文献,通过统一的文献纳入标准、排除标准,对每篇文献进行评价。将每篇有效文献的穴位输入Excel表中并进行合并和排序,采用分层聚类统计方法对前20位推拿手法进行分析。结果:治疗膝关节骨性关节炎使用频次最高前20位的手法依次为:揉法、拿捏法、按法、屈伸膝法、点压法、㨰法、推法、弹拨法、摩擦法、牵拉法、理顺法、旋转法、一指禅推法、摇膝法、叩击法、提膝法、放松手法、搓法、扳法、拔伸拨络法。结论:从手法作用部位来看:以作用于人体软组织类手法为主,以作用于人体骨与关节类手法为辅。从手法发力特点来看:①作用于人体软组织类手法中,又以摆动类手法为主,以挤压类手法为辅;②作用于骨与关节类手法又以运动关节类手法为主。Objective:To discuss the principle of massage manipulation in curing of knee osteoarthritis in recent ten years literature.And provide the reference and theoretical direction of the method in massage manipulation of curing knee osteoarthritis.Methods:First through the common retrieval type in WANFANG,WEIPU and CNKI.Second through the inclusion criteria,exclusion criteria,evaluation of literature quality,data fetch and sort out the retrieval result.We choose acupoints from valid literatures and type in Microsoft Excel to rank.Use statistical method of hierarchical clustering and analyze the top 20 massage manipulations.Results:Final valid literatures for the 658,ranked in the top 20 practices were:kneading method,take the kneading method,according to law,flexion and extension knee method,point pressure method,roll method,push method,plucking method,friction method,Straightening method,rotation method,a finger push method,shake knee method,percussion method,knee method,relaxation method,rubbing method,pull method,pull the dial method.Conclusion:From the point of viewof manipulation:to act on the human soft tissue type of manipulation,to act on the human bone and joint type of manipulation.From the point of viewof the characteristics of manual force:1 for human soft tissue type of manipulation,mainly swing type of manipulation,with compression type of manipulation as a supplement;2 for bone and joint type of manipulation and sports joint type of manipulation.
