综合护理对机械通气脱机成功率的影响

摘　　要：目的分析综合护理对重症肺炎合并呼吸衰竭患者脱机成功率的影响。方法选择2018年1月-2019年2月广东省农垦中心医院收治的重症肺炎合并呼吸衰竭住院患者60例。根据随机数字表法分为对照组和观察组各30例,对照组行常规护理,观察组行综合护理。比较两组脱机成功率、人机对抗成功率及住院时间。结果观察组一次脱机成功率为63.33%,明显高于对照组的36.67%,差异有统计学意义(P<0.05)。观察组两次脱机成功率、多次脱机不成功/呼吸机依赖分别为20.00%、16.67%,与对照组的26.67%、36.67%比较,差异无统计学意义(P>0.05)。观察组人机对抗率及住院时间分别为13.33%、(10.42±1.16)d,均低于对照组的40.00%、(18.42±2.14)d,差异有统计学意义(P<0.05)。结论综合护理可有效改善重症肺炎合并呼吸衰竭患者预后,提升患者脱机成功率,减少人机对抗情况发生,缩短住院时间,促使患者早日康复。Objective To analyze the effect of comprehensive nursing on the off-line success rate of patients with severe pneumonia combined with respiratory failure. Methods From January 2018 to February 2019,60 inpatients with severe pneumonia combined with respiratory failure were selected. According to the random number table method,the patients were divided into control group and observation group,30 cases in each group. The control group received routine nursing, while the observation group received comprehensive nursing. The off-line success rate, man-machine confrontation success rate and hospitalization time were compared between the two groups. Results The success rate of one off-line in the observation group was 63.33%,which was significantly higher than that in the control group with 36.67%,the difference was statistically significant(P<0.05). The success rate of two off-line and multiple off-line unsuccessful/ventilator dependence in the observation group were 20.00% and 16.67%,respectively,compared with 26.67% and 36.67% in the control group,the differences were not statistically signif icant(P>0.05). The man-machine confrontation rate and hospitalization time in the observation group were 13.33%and(10.42±1.16)days,respectively,lower than those in the control group with 40.00% and(18.42 ±2.14)days,the differences were statistically significant(P<0.05). Conclusion Comprehensive nursing could effectively improve the prognosis of patients with severe pneumonia combined with respiratory failure,enhance the success rate of patients off-line,reduce the occurrence of man-machine confrontation,shorten the hospitalization time,and promote the early recovery of patients.
