中医综合外治法对肛瘘术后模型大鼠创面愈合指标的影响

摘　　要：目的:探讨中医综合外治法对肛瘘术后模型大鼠创面愈合的机制。方法:选定30只SD雄性大鼠按随机数字表法分为中医综合外治组(治疗组)、口服曲马多组(对照组)、空白对照组(空白组),对创面组织采用Western Blot法检测血管内皮生长因子(VEGF)、成纤维细胞生长因子(bFGF),采用ELISA法检测白介素-1(IL-1)、白介素-6(IL-6)、肿瘤坏死因子(TNF-α),PCR法检测bFGFmRNA、神经肽P物质mRNA(SPmRNA)、NK1RmRNA。结果:与空白组比较,治疗组与对照组的bFGF及其mRNA、VEGF、SPmRNA、NK1RmRNA均升高,IL-1、IL-6、TNF-α均降低;与对照组比较,治疗组的VEGF、SPmRNA、NK1RmRNA均升高,IL-1、IL-6、TNF-α均降低。结论:中医综合外治法可促进创面愈合,效果优于口服曲马多治疗,并可通过上调SP/NK1R促进bFGF、VEGF表达,抑制L-1、IL-6、TNF-α表达,加快肉芽组织血管生成并降低炎症反应。Object:To explore the mechanism of TCM comprehensive external treatment on wound healing in rats after anal fistula operation.Methods:Thirty SD male rats were randomly divided into comprehensive external treatment of TCM group(treatment group),oral tramadol group(control group)and blank control group(blank group).Vascular endothelial growth factor(VEGF)and fibroblast growth factor(bFGF)were detected by Western Blot on wound tissue.Interleukin-1(IL-1),interleukin-6(IL-6)and tumor necrosis factor(TNF-α)were detected by Elisa;bFGF mRNA,neuropeptide substance P mRNA(SP mRNA),and NK1R mRNA were detected by PCR.Results:Compared with the blank group,bFGF and its mRNA,VEGF,SPmRNA and NK1R mRNA were increased in the treatment group and the control group,and IL-1,IL-6 and TNF-αwere decreased;compared with the control group,the VEGF,SPmRNA and NK1R mRNA of the treatment group were increased,IL-1,IL-6 and TNF-αwere decreased,bFGF and its mRNA were increased.Conclusion:The comprehensive external treatment of TCM can promote wound healing.The effect is better than oral tramadol treatment,and could probably up-regulate SP/NK1R,promote the expression of bFGF and VEGF,inhibit the expression of IL-1,IL-6 and TNF-α,accelerate angiogenesis in granulation tissue and reduce inflammatory response.
