参附注射液联合益气活血合剂治疗老年多器官功能不全综合征的临床观察

摘　　要：目的:探讨参附注射液联合益气活血合剂对老年多器官功能不全患者临床疗效观察。方法:将180例患者采用随机数字表法分为治疗组92例和对照组88例。患者入院后均按西医常规对症支持处理,治疗组在此基础上给予参附针联合益气活血合剂治疗,以14 d为1个疗程,治疗1个疗程比较2组临床疗效,并对2组治疗前后的APACHEⅢ量表评分及阳虚血瘀证型评分进行对比分析,比较2组治疗后的死亡率及2组治疗前后炎症因子变化。结果:治疗组显效率、总有效率分别为71.74%、86.95%,对照组显效率、总有效率分别为36.47%、75%,2组显效率及总有效率差异有统计学意义;治疗组和对照组APACHEⅢ治疗后积分较治疗前明显降低,差异有统计学意义,治疗组和对照组阳虚血瘀证型治疗后积分较治疗前明显降低,差异有统计学意义;治疗组死亡率明显低于对照组,治疗后其死亡率比较差异有统计学意义;2组患者治疗后炎症因子血清hs-CRP较治疗前均有下降,与本组治疗前比较差异有统计学意义。结论:参附注射液联合益气活血合剂对老年多器官功能不全具有改善作用,可以降低多器官功能不全患者死亡率,有效减少炎症因子。Objective:To investigate the clinical effect of Shenfu injection combined with YiqiHuoxue Mixture on patients with multiple organ dysfunction syndrome in the Elderly(MODSE).Method:Using randomized controlled methods,180 patients wererandomly divided into treatment group(n=92)and control group(n=88).All patients were treated with conventional western medicine,while treatment group was added with Shenfu Injection with YiqiHuoxue Mixture,and course of treatment was 10 days for two groups.Clinical efficacy,acute physiology and chronic health evaluationⅢ(APACHEⅢ)score and Yang Deficiency and Blood Stasis Syndrome score before and after treatment were compared in two groups.The mortality rate and the changes of inflammatory factors of two groups was compared after one course of treatment and two courses of treatment.Result:The marked efficiency and total effective rate of the treatment group were 71.74%and 86.95%,and the marked efficiency and total effective rate of the control group were 36.47%and 75%.Respectively,the difference was statistically significant;APACHE III scores before and after treatment in treatment group and control group were significantly lower than those before treatment;Yang Deficiency and Blood Stasis Syndrome scores before and after treatment in treatment group and control group were significantly lower than those before treatment;The mortality rates of the two groups before and after the first and second courses of treatment were compared,there was a difference in the mortality rates between the two groups;After treatment,the levels of hs-CRP and TNF-a in serum of patients in both groups were lower than those before treatment,the difference was statistically significant.Conclusion:Shenfu Injection combined with Yiqihuoxue Mixture can improve the functions,reduce the mortality rate of patients with MODSE and effectively reduce inflammatory factors.
