摘　　要：目的对医院医疗质量进行综合评价及分析,分析存在的问题,为更好地实现医疗质量管理提供参考。方法应用TOPSIS法对某三级甲等肿瘤专科医院住院医疗质量进行综合评价,计算评价对象与最优方案的接近程度Ci值,Ci值愈接近1,表明该评价对象医疗质量越高;应用多元线性回归方法探寻综合评价结果的影响因素。结果该院2017年3月份医疗质量最高,Ci值为0.7999;1月份医疗质量最低,Ci值为0.2318,差异较大;多元线性回归分析结果显示,出院人数、床位使用率及院内感染发生率是住院医疗质量的主要影响因素(P<0.001);此外,经医疗质量目标管理考核方案质控数据可知,2017年考核成绩较好的月份是11月、10月及12月。结论TOPSIS法在医疗质量评价中具有一定的实际应用价值,建议将TOPSIS法与医院管理实践相结合,客观、真实地反映医院医疗质量;此外,医院应协调推进工作强度、工作效率以及诊疗水平,注重质量并兼顾效率。Objective To comprehensively evaluate and analyze the medical quality of the hospital,analyze the exis-ting problems,and provide a reference for better realization of medical quality management.Methods TOPSIS method was used to comprehensively evaluate the inpatient medical quality of a tertiary A hospital,and the Ci value of the evaluation object and the optimal plan was calculated.The closer the Ci value is to 1,the higher the medical quality of the evaluated object.The multiple linear regression method was used to explore the comprehensive influencing factors of evaluation results.Results The hospital's medical quality was the highest in March 2017,with a Ci value of 0.7999;the medical quality was the lowest in January,with a Ci value of 0.2318,with great difference.The results of multiple linear regression analysis showed that the number of discharged patients,bed utilization rate,and hospital infection rate were the main influencing factor of inpatient medical quality(P<0.001).In addition,according to the quality control data of the medical quality target management assessment program,the 2017 assessment results were better in November,October and December.Conclusion The TOPSIS method has certain practical application value in the evaluation of medical quality.It is recommended to combine the TOPSIS method with hospital management practices to objectively and truly reflect the medical quality of the hospital.In addition,the hospital should coordinate the promotion of work intensity,work efficiency and diagnosis and treatment level and focus on quality and efficiency.
