摘　　要：恶性血液病是指各型急慢性白血病、淋巴瘤、骨髓增生异常综合征、骨髓增殖性肿瘤、多发性骨髓瘤等血液系统恶性肿瘤。化疗是大多数恶性血液病的常规治疗手段,但化疗药物在杀伤肿瘤细胞的同时也会杀伤人体的正常细胞,引起一系列不良反应的发生。胃气是中医基础理论的一个重要概念,脾胃为"后天之本",气血生化之源,气机升降之枢纽,且与他脏有密切联系,而胃气是脾胃功能的重要体现,探讨"有胃气则生"理论在临床中的应用有重要意义。脾胃亏虚是恶性血液病化疗患者的重要病因,是肿瘤发病的重要因素,化疗导致恶性血液病患者脾胃虚弱加重。调理脾胃、顾护胃气在恶性血液病化疗中具有重要作用。化疗前期,以理气祛邪为主,兼以顾护胃气;化疗期,扶正祛邪,调理脾胃;化疗后期,扶正培本,恢复胃气。中西医结合疗效更佳。临床应结合辨证遣药组方,药物宜纯、宜平,分量宜轻,避免使用大辛大热、大苦大寒、大补大泻等伤脾败胃之品。Malignant hematopathy refers to various types of acute and chronic leukemia, lymphoma, myelodysplastic syndrome, myeloproliferative tumor, multiple myeloma and other hematological malignancies. Chemotherapy is a conventional treatment for most hematological malignancies, but chemotherapy drugs kill tumor cells as well as normal cells, causing a series of adverse reactions. Stomach qi is an important concept in the basic theory of traditional Chinese medicine. The spleen and stomach are the foundation of postnatal existence, the sufficient source of qi and blood, the hinge of Qi rising and falling, and closely related to other organs. Stomach qi is an important embodiment of the functions of the spleen and stomach. Thus, it is of great significance to explore the application of the theory of "Stomach gives life" in clinical practice. Deficiency of the spleen and stomach is an important cause of hematologic malignancies in chemotherapy patients and an important factor in the pathogenesis of tumors. Chemotherapy leads to the aggravation of the spleen and stomach weakness in patients with hematologic malignancies. Regulating the spleen and stomach and protecting stomach qi play an important role in chemotherapy of malignant hematological diseases. In the early stage of chemotherapy, treatment should be centered on regulating qi and eliminating pathogenic factors, and also on protecting the stomach qi;during chemotherapy, it should be centered on strengthening the body resistance to eliminate pathogenic factors and regulating the spleen and stomach;in the later stage of chemotherapy, it should be centered on strengthening the healthy qi and restoring stomach qi. The combination of traditional Chinese and Western medicine is more effective. In clinical practice, it is necessary to prescribe according to syndrome differentiation. The medicinals should be pure, flat in property and light in quantity, so as to avoid the use of drugs that hurt the spleen and stomach, such as medicinals greatly heat and pungent in property,
