植入式静脉输液港对淋巴瘤化疗患者预后生活质量的效果分析

摘　　要：目的了解植入式静脉输液港对淋巴瘤化疗患者预后生活质量的效果,为相关临床治疗提供数据参考。方法采用随机数字表法将160例淋巴瘤患者分为观察组和对照组。对照组患者(80例)实施外周静脉置入中心静脉导管(PICC)进行输液;观察组患者(80例)实施植入式静脉输液港(IVAP)进行输液;并对两组患者的导管留置时间、管路维护时间、一次性穿刺成功率、意外拔管率、并发症发生率和生活质量进行比较。结果对照组的管路维护时间长于观察组,导管留置时间短于观察组,差异具有统计学意义(P<0.01)。化疗后,两组患者的社会方面、精神及心理方面、生理方面、其他方面和生活质量总分均高于化疗前,差异均具有统计学意义(P<0.05);化疗后除了其他方面外,观察组的社会方面、精神及心理方面、生理方面和生活质量总分均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率及意外拔管率均低于对照组,差异具有统计学意义(P<0.05)。结论相对于PICC,IVAP能更好地延长导管留置时间、缩短管路维护时间,降低并发症发生率等,能更好地提高患者的生活质量。【Objective】To observe the effect of implantable venous access port(IVAP)in improving the quality of life of patients with lymphoma chemotherapy,providing data support for clinical treatment.【Methods】A total of 160 patients with lymphoma chemotherapy were randomly divided in to observation group(80 cases)and control group(80 cases).The control group was treated with peripherally inserted central catheter(PICC),and the observation group received implantable venous access port infusion.The duration of catheter maintenance,pipeline maintenance time,one-time puncture success rate,accidental extubation rate,complication rate,and quality of life were compared between the two groups.【Results】The duration of catheter maintenance of the control group was longer than that of the observation group,and the pipeline maintenance time was shorter than that of the observation group,and the difference was statistically significant(P<0.01).After chemotherapy,the total scores of social aspects,mental and psychological aspects,physiological aspects,other aspects and quality of life of patients in the two groups were higher than those before chemotherapy,and the differences were statistically significant(P<0.05).Except other aspects after chemotherapy,the total scores of social aspects,mental and psychological aspects,physiological aspects and quality of life in the observation group were all higher than those in the control group,with statistically significant differences(P<0.05).The incidence of complications and accidental extubation rate in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).【Conclusion】Compared with PICC,IVAP can better prolong the duration of catheter maintenance,shorten the time of pipeline maintenance,reduce the incidence of complications,and improve the quality of life of patients.
