舒肝解郁通络汤对前部缺血性视神经病变患者视力、眼血流动力学及PVEP的影响

摘　　要：目的:探讨舒肝解郁通络汤治疗前部缺血性视神经病变(AION)的疗效及对患者视力、眼血流动力学及图形视觉诱发电位(PVEP)的影响。方法:选取符合纳入标准的AION患者88例,按随机数字表分为观察组(43例)与对照组(45例)。对照组在常规治疗的基础上给予静脉滴注低分子右旋糖酐氨基酸注射液治疗,500 mL/次,1次/d;观察组在对照组的基础上给予舒肝解郁通络汤治疗,每日1剂,2次/d。两组患者均连续治疗14 d。疗程结束后,比较治疗前后2组患者视力、眼血流动力学及PVEP的差异和治疗期间不良反应发生情况。结果:与治疗前比较,治疗后观察组患者视力指数0.1~0.3的患者占比降低,视力指数0.7~0.9与≥1.0的患者占比升高,且观察组视力指数0.7~0.9与≥1.0的患者占比显著高于对照组(P<0.05);与治疗前比较,治疗后2组患者全血低切黏度、全血高切黏度及纤维蛋白原水平均降低,且观察组明显低于对照组(P<0.05);与治疗前比较,两组患者振幅(AP100)升高,且观察组高于对照组(P<0.05);潜伏期(LP100)缩短,且观察组短于对照组(P<0.05);MD降低,观察组低于对照组(P<0.05);治疗期间,对照组不良反应发生率为8.89%(4/45),观察组不良反应发生率为6.98%(3/43),两组比较差异无统计学意义(P>0.05)。结论:舒肝解郁通络汤可明显改善AION患者血液流变学与PVEP,提高患者视力水平,且安全有效,值得临床推广应用。Objective:To investigate the effects of Shugan Jieyu Tongluo Decoction on visual acuity,ocular hemodynamics and pattern visual evoked potential(PVEP)in patients with anterior ischemic optic neuropathy(AION).Methods:88 cases of AION were randomly divided into the observation group(n=43)and the control group(n=45).The control group was given intravenous low-molecular-dextran amino acid injection on the basis of conventional treatment;the observation group was treated with Shugan Jieyu Tongluo Decoction on the basis of the treatment of the control group.After 14 days of continuous treatment,visual acuity,ocular hemodynamics,and PVEP were compared between the two groups.Adverse reactions during treatment were also observed in the two groups.Results:Compared to that before the treatment,the proportion of patients with visual index of 0.1-0.3 decreased and the proportion of patients with 0.7-0.9 or higher than 1.0 increased after the treatment in the observation group;of which the improvements were more significant in the observation group than those in the control group(P<0.05).The low-cut viscosity of whole blood,high-cut viscosity of whole blood,and fibrinogen levels were decreased after the treatment in the two groups,and the decreases were more significant in the observation group(P<0.05).After the treatment in the two groups,the amplitude(AP100)were increased,of which the observation group was higher than the control group(P<0.05);the incubation period(LP100)was shortened,and the observation group was shorter than the control group(P<0.05);the MD was decreased,and the observation group decreased more significantly than the control group(P<0.05).In terms of the incidence of adverse reactions,it was 8.89%(4/45)in the control group and 6.98%(3/43)in the observation group,there was no statistical difference between the two groups(P>0.05).Conclusion:Shugan Jieyu Tongluo Decoction can significantly improve the hemodynamics and PVEP of patients with AION,it can improve the patient's vision acuity.The therapy is safe and
