揿针治疗周围性面瘫随机对照试验的Meta分析

摘　　要：目的评价揿针治疗周围性面瘫的有效性和安全性。方法计算机检索国内外医学数据库有关揿针治疗周围性面瘫的临床随机对照研究,时间限定为建库至2019年12月1日,根据纳入标准、排除标准选择文献并提取数据信息,参照Cochrane系统评价手册,对纳入的研究进行方法学质量评价,运用RevMan 5.3软件进行Meta分析。结果共纳入12项研究,共计1 112名周围性面瘫患者,纳入文献研究质量等级较低。Meta分析结果显示:8项研究通过总有效率进行差异比较,RR=1.18,95%CI:(1.12~1.25),Z=5.69,P<0.001;3项研究通过愈显率进行比较,RR=1.28,95%CI:(1.03~1.58),P=0.02;2项研究通过面神经分级量表(H-B)比较,MD=-1.18,95%CI:(-1.59^-0.77),P<0.001,揿针结合对照组疗效优于单纯对照组,差异有统计学意义。结论揿针辅助治疗周围性面瘫具有较好的临床疗效,优于非揿针结合治疗方案组。Objective To evaluate the efficacy and safety of intradermal needle in the treatment of peripheral facial paralysis.Methods A computer search of domestic and foreign medical databases for clinical randomized controlled trials of intradermal needle in the treatment of peripheral facial paralysis was conducted until December 1,2019.Literature was selected according to the inclusion and exclusion criteria and data were extracted.The included studies were evaluated for methodological quality by referring to the Cochrane systematic review manual,and the RevMan 5.3 software was used for meta-analysis.Results A total of 12 studies were included,including 1 112 patients with peripheral facial paralysis.The quality of the included literature was lower.The results of meta-analysis showed that:eight studies compared total effective rate,RR=1.18,95%CI:(1.12-1.25),Z=5.69,P<0.001;three studies compared good improvement rate,RR=1.28,95%CI:(1.03-1.58),P=0.02;two studies compared facial nerve grading scale(H-B),MD=-1.18,95 % CI:(-1.59--0.77),P<0.001.The therapeutic effect of intradermal needle combined with the control was better than that of the control group alone,and the difference was statistically significant.Conclusion Intradermal needle has a better clinical effect in the treatment of peripheral facial paralysis,which is better than non-intradermal needle treatment.
