低剂量帕罗西汀治疗围绝经期女性血管舒缩症状的临床疗效观察

摘　　要：目的探讨低剂量帕罗西汀治疗围绝经期女性血管舒缩症状的临床疗效与安全性。方法2018年1月至2019年12月四川省人民医院门诊就诊的79例围绝经期女性,改良Kupperman评分为中到重度,分为盐酸帕罗西汀治疗组39例(10 mg/d,1日1次),激素治疗组40例(雌孕激素序贯治疗方案:17-β雌二醇1 mg14片+17-β雌二醇1 mg地屈孕酮10mg14片,1片/天)。两组均连用12周。比较治疗后2、4、8、12周两组患者的Kupperman评分和血管舒缩症状,12周后用药的不良反应。结果治疗第2、4、8、12周时,两组患者潮热出汗的频率差异均无统计学意义(P>0.05),治疗第12周两组潮热出汗频率较治疗前均有明显改善(P<0.05);两组患者Kupperman评分均低于同组治疗前,激素治疗组评分低于盐酸帕罗西汀治疗组(P<0.05)。两组患者不良反应总发生率比较差异无统计学意义(P>0.05)。结论低剂量(10 mg)盐酸帕罗西汀在治疗围绝经期女性血管舒缩症状的频率方面与激素治疗具有相似的效果,对改善其他围绝经期症状及血管舒缩症状程度方面稍差于激素替代治疗,其耐受性好,安全有效,是围绝经期女性改善症状较好的非激素治疗方法。Objective To explore the efficacy and safety of low dose paroxetine in the treatment of menopausal vasomotor symptoms.Methods From January 2018 to December 2019,79 postmenopausal women at outpatient department in our hospital were randomly divided into paroxetine hydrochloride(10 mg per day)treatment group(n=39)and hormone(1 tablet per day)treatment group(n=40).The hormone sequential therapy was 14 tablets of 17βestradiol 1 mg plus 14 tablets of 17βestradiol 10 mg,1 table a day.Both groups were treated for 12 weeks.Kupperman score and the frequency of vasomotor symptoms were assessed after 2,4,8 and 12 weeks of treatment.Adverse reactions were assessed after 12 weeks of treatment.Results At the 2 nd,4 th,8 th and 12 th week of treatment,there was no significant difference in the frequency of hot flashes and sweating between the two groups(P>0.05).The frequency of hot flashes and sweating at the 12 th week of treatment was significantly improved compared with that before treatment(P<0.05).After treatment,Kupperman score of the two groups was lower than that before treatment,and the score of hormone treatment group was lower than that of paroxetine hydrochloride treatment group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Low dose paroxetine hydrochloride(10 mg)has similar effect to hormone therapy in the frequency of postmenopausal women’s vasomotor symptoms.It may be slightly worse than hormone replacement therapy in improving other postmenopausal symptoms and the degree of vasomotor symptoms.However,paroxetine is well tolerated,safe and effective.It is a good non-hormone therapy method for postmenopausal women to improve their symptoms.
