摘 要:目的探讨围手术期白蛋白、ASA分级对骨科患者术后1期压力性损伤的预测价值。方法选取本院325例骨科压力性损伤高风险患者,将其分为1期压力性损伤组(n=31)和非1期压力性损伤组(n=294),分析影响患者术后发生1期压力性损伤的危险因素,比较两组围手术期白蛋白、ASA分级差异。结果325例骨科患者术后发生1期压力性损伤31例,发生率9.54%;发生1期压力性损伤与不发生1期压力性损伤患者年龄、围手术期白蛋白水平、ASA分级、手术时间、手术体位、术中输血情况及皮肤类型比较差异有统计学意义(P<0.05),性别、麻醉类型比较差异无统计学意义(P>0.05);年龄≥80岁、围手术期白蛋白<35 g/L、ASA分级≥Ⅲ级、术中输血是影响骨科患者术后1期压力性损伤的独立危险因素(P<0.05);1期压力性损伤组围手术期白蛋白水平低于非1期压力性损伤组,ASA分级Ⅲ级比例高于非1期压力性损伤组(P<0.05)。结论术后发生1期压力性损伤的骨科患者围手术期白蛋白水平较低,ASA分级Ⅲ级比例较高,围手术期白蛋白、ASA分级对骨科患者术后1期压力性损伤具有一定的预测价值。Objective To explore the predictive value of perioperative albumin and ASA classification for postoperative stage 1 stress injury of orthopedic patients.Methods A total of 325 patients with high risk of orthopedic pressure injury in our hospital were selected.They were divided into stage 1 stress injury group(n=31)and non-stage 1 stress injury group(n=294).The risk factors of postoperative stage 1 stress injury were analyzed.The differences in perioperative albumin and ASA classification were compared between the two groups.Results Among the 325 patients,there were 31 cases with postoperative stage 1 stress injury and the incidence rate was 9.54%.There were significant differences in the age,perioperative albumin level,ASA classification,surgical time,surgical position,intraoperative blood transfusion and skin type between the two groups(P<0.05)while there were no significant differences in the gender and anesthesia type between the two groups(P>0.05).Age≥80 years old,perioperative albumin<35 g/L,ASA classification≥grade III and intraoperative blood transfusion were independent risk factors for postoperative stage 1 stress injury in orthopedic patients(P<0.05).Perioperative albumin level was significantly lower while the proportion of patients with ASA grade III was significantly higher in the stage 1 stress injury group than those in the non-stage 1 stress injury group(P<0.05).Conclusion The perioperative albumin level is lower and the proportion of ASA grade III is higher in patients with postoperative stage 1 stress injury when compared to patients without stage 1 stress injury.The perioperative albumin and ASA classification have a certain predictive value for postoperative stage 1 stress injury in orthopedic patients.
