原发性干燥综合征的口腔微生态失调的护理对策

摘　　要：目的本研究旨在对比针对性口腔护理与传统常规护理在原发性干燥综合征(primary Sjo⁃gren′s syndrome,PSS)患者口腔微生态及口腔健康状况改善中的应用效果,为PSS患者口腔疾病的预防与护理提供指导。方法选取2018年5月1日至2018年10月31日于温州医科大学附属第一医院风湿免疫科收治的PSS患者70例,随机分为观察组和对照组,对照组进行常规护理,观察组进行实施针对性口腔护理。通过口腔健康影响程度量表(OHIP⁃14)以及16S测序观察两组护理前后口腔健康情况及口腔微生态特征。结果两组OHIP⁃14评分均较入院时降低,且观察组降低幅度明显大于对照组,差异有统计学意义(P<0.05)。微生物群落分析发现观察组的物种多样性显著大于对照组,且两组之间的微生物组成具有明显差异。结论针对性的口腔护理法在PSS患者的口腔微生态失调下的应用效果优于常规护理,且可以改善PSS患者的口腔微生物群落多样性。Objective The aim of this study is to compare the effect of specific with traditional oral care in the improvement of oral microecology and oral health status of primary Sjogren′s syndrome(PSS)patients,which may provide guidance for the prevention and nursing of oral diseases in PSS patients.Methods A total of 70 PSS patients were recruited from the First Affiliated Hospital of Wenzhou Medical University Rheumatology Department and randomly divided into the observation and control group(35 each group).Regular carewas performed in the control group and specific oral care in the observation group respectively.Oral health and micro⁃ecological features were detected before and after nursing by using Oral Health Impact Profile⁃14(OHIP⁃14)and 16S sequencing.Results The OHIP⁃14 of both two groups were significantly decreased when comparing with admission time.However,the decrease in the observation group was significantly larger than that in the control group.Microbial community analysis revealed that higher level of species diversities were observed than control group.There were significant differences in microbial composition between two groups.Conclusion Application of specific oral care is better than regular care and it improves the diversity of oral microbial community in PSS patients.
