胃大部切除术与腹腔镜胃十二指肠溃疡穿孔修补术治疗胃十二指肠溃疡穿孔疗效比较

摘　　要：目的比较胃大部切除术与腹腔镜胃十二指肠溃疡穿孔修补术治疗胃十二指肠溃疡穿孔的疗效。方法选取2016年2月~2017年2月85例在我院住院的胃十二指肠溃疡穿孔患者,根据手术方法不同分为观察组(腹腔镜胃十二指肠溃疡穿孔修补术)43例和对照组(胃大部切除术)42例。比较两组的各种手术指标、并发症发生情况及6、12个月溃疡复发率。结果观察组患者的平均手术时间、术中出血量、住院时间明显低于对照组(P<0.05),术后开始下床时间明显早于对照组,差异有统计学意义(P<0.05)。观察组患者的术后并发症率显著低于对照组(4.65%vs 23.81%),差异有统计学意义(P<0.05)。观察组患者6、12个月溃疡复发率分别为4.65%、13.95%,低于对照组的7.14%、16.67%,两组比较差异无统计学意义(P>0.05)。结论腹腔镜胃十二指肠溃疡穿孔修补术治疗胃十二指肠溃疡穿孔疗效显著,可明显减少出血量,患者可早日下床活动,并发症发生率低,有利于患者术后病情的及早恢复,值得临床推广应用。Objective To compare the curative effect between subtotal gastrectomy and laparoscopic repair of perforation of gastroduodenal ulcer.Methods 85 patients with perforation of gastroduodenal ulcer treated in our hospital from February 2016 to February 2017 were divided into observation group(using Laparoscopic repair of perforation of gastroduodenal ulcer,n=43)and control group(using subtotal gastrectomy,n=42)according to different surgical methods.The operation indexes,complications and recurrence rate of ulcer in 6 and 12 months were compared between the two groups.Results The average operation time,intraoperative blood loss and hospitalization time in the observation group were significantly lower than those in the control group(P<0.05),and the time of getting out of bed after operation was significantly earlier than the control group,and the differences were statistically significant(P<0.05).The postoperative complication rate of the observation group was significantly lower than the control group(4.65%vs 23.81%),and the difference groups was statistically significant(P<0.05).The recurrence rate of ulcer in 6 and 12 months in the observation group was 4.65%,13.95%,respectively,which was lower than the control group(7.14%,16.67%),and the difference was not statistically significant between two groups(P>0.05).Conclusion Laparoscopic repair of perforation of gastroduodenal ulcer is effective in the treatment of perforation of gastroduodenal ulcer.It can reduce the amount of bleeding,shorten the time of getting out of bed,and lower the incidence of complications.It is beneficial to the early recovery of patients',and is worthy of clinical application.
