补肾养血方对薄型子宫内膜不孕患者雌二醇水平与血流阻力指数及子宫内膜形态的影响

摘　　要：目的探讨补肾养血方对薄型子宫内膜不孕患者雌二醇(E2)水平与血流阻力指数(RI)及子宫内膜形态的影响。方法选取2016年3月~2018年3月本院收治的薄型子宫内膜不孕患者216例,按照随机数字表法分为观察组与对照组,各108例,对照组采用芬吗通治疗,观察组采用补肾养血方联合芬吗通治疗,比较两组治疗前后子宫内膜厚度、血流阻力指数(RI)、生存质量量表(QOL)评分、子宫内膜形态、E2、黄体生成素(LH)、血清孕酮、1年内妊娠率及妊娠结局的差异。结果治疗前,两组患者子宫内膜厚度、RI指数、QOL评分、子宫内膜形态、E2、LH、血清孕酮水平相比,无显著差异(P>0.05);治疗后,两组患者子宫内膜厚度、E2及血清孕酮水平较治疗前明显升高,RI指数、QOL评分、LH水平较治疗前明显降低(P<0.05);治疗后,观察组子宫内膜形态A型比例较治疗前明显增加,C型比例较治疗前明显降低(62.96%vs 17.59%)(48.15%vs 13.89%)(P<0.05);观察组患者1年内妊娠率明显高于对照组(53.70%vs 31.48%)(P<0.05);观察组有2例流产,对照组有3例流产,余患者均足月分娩。结论补肾养血方联合芬吗通治疗薄型子宫内膜不孕具有较好疗效,能有效增加子宫内膜厚度,改善雌二醇等内分泌激素水平,降低血流阻力指数,提高妊娠率,值得临床大力推广。Objective To investigate the effect of Bushen Yangxue recipe on estradiol(E2)level,blood flow resistance index(RI)and endometrial morphology in patients with thin endometrial infertility.Methods A total of 216 patients with thin endometrial infertility admitted to our hospital from March 2016 to March 2018 were randomly divided into observation group and control group according to the digital table method,with 108 cases in each group.The control group was treated with femonston.The observation group was treated with Bushen Yangxue recipe combined with femonston.The differences in endometrial thickness,blood flow resistance index(RI),quality of life scale(QOL)score,endometrial morphology,E2,luteinizing hormone(LH),serum progesterone,pregnancy rate within 1 year,and pregnancy outcome were compared before and after treatment.Results Before treatment,there were no significant differences in endometrial thickness,RI index,QOL score,endometrial morphology,E2,LH,and serum progesterone levels between the two groups(P>0.05).After treatment,the endometrial thickness,E2 and serum progesterone levels in the two groups of patients were significantly higher than those before treatment.The RI index,QOL score and LH level in two groups were significantly lower than those before treatment(P<0.05).After treatment,the proportion of endometrial morphology A in the observation group was higher than that before treatment(62.96%vs 17.59%),and the proportion of type C was significantly lower than that before treatment(48.15%vs 13.89%)(P<0.05).The pregnancy rate of the observation group was significantly higher than that of the control group within 1 year(53.70%vs 31.%)(P<0.05).There were 2 cases of miscarriage in the observation group,3 cases of abortion in the control group.And the remaining patients were delivered at full term.Conclusion Bushen Yangxue recipe combined with femonston has a good curative effect on thin endometrial infertility,which can effectively increase endometrial thickness,improve endocrine hormone levels such as e
