应用护理质量指标提高肠道准备质量的实践

摘　　要：目的探讨在肠道准备过程中实施护理质量指标管理的效果。方法根据《中国消化内镜诊疗相关肠道准备指南》及肠道准备关键流程制订了7项护理质量指标(包括肠道准备评估准确率、患者肠道准备知识知晓率、护士肠道准备相关知识考核合格率、肉眼肠道清洁合格率、内镜下肠道清洁合格率、医生对肠道准备满意度、患者对肠道准备满意度),并建立了阈值,应用于2018年1月至12月在南京医科大学第一附属医院消化内科行肠镜检查或治疗的患者中,定期统计护理质量指标,并将结果与阈值进行比较,进行持续质量改进。结果统计2018年12月份行肠镜检查或治疗的1446例患者上述各项指标显示,上述7项护理质量指标分别达到97%(>95%)、95%(>90%)、100%(100%)、92%(>90%)、88%(>85%)、95%(>90%)、94%(>90%),均达到或超过了阈值范围。结论护理质量指标应用于肠道准备管理,能够提高肠道准备护理质量、护士核心能力和医患满意度。Objective To evaluate the effect of nursing quality indicators for bowel preparation.Methods Seven quality indicators for bowel preparation were applied based on the Practice Guideline of Bowel Preparation including the accuracy of bowel preparation,awareness rate of bowel preparation,skills of nurses,acceptability of intestinal cleaning,acceptability of endoscopic intestinal cleaning,the satisfaction for bowel preparation.The cut-off value was evaluated and applied to the patients receiving colonoscopy from January to December 2018 in Gastroenterology Department of the First Affiliated Hospital of Nanjing Medical University.The data on the seven quality indicators were collected and compared to the cut-off values regularly.Results The data collected from 1446 patients who received colonoscopy in December showed that all of nursing quality indicators in bowel preparation reached the threshold ranges.Conclusions The application of nursing quality indicators can improve the quality of care in bowel preparation and the core competence of nurses as well as satisfaction of patients and doctors.
