摘  要:目的探究微型切口锁定钢板内固定术对SandersⅡ型跟骨骨折患者术后疼痛程度及AOFAS评分的影响。方法选取我院2015年9月至2018年9月收治的90例SandersⅡ型跟骨骨折患者,根据手术方案的不同将其分为对照组和观察组,各45例。对照组给予L形切口解剖型钢板内固定治疗,观察组给予微型切口锁定钢板内固定术治疗。比较两组手术相关指标、疼痛评分、足部功能恢复情况、跟骨内翻角及并发症发生情况。结果观察组的术中出血量少于对照组,手术时间、住院时间均短于对照组(P<0.05)。观察组术后1 d、术后1周、术后1个月的VAS评分均低于对照组(P<0.05)。观察组术后3个月的AOFAS评分高于对照组及术前(P<0.05)。术前、术后3个月,两组患者的跟骨内翻角无显著差异(P>0.05);两组患者术后3个月的跟骨内翻角均显著减小(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论微型切口锁定钢板内固定术治疗SandersⅡ型跟骨骨折患者,具有创伤小、手术时间短、并发症少、术后疼痛轻等优势,可促进术后恢复,提高AOFAS评分,有效复位跟骨力线。Objective To investigate the effects of micro-incision locking plate internal fixation on postoperative pain and AOFAS scores in patients with Sanders type Ⅱ calcaneal fractures. Methods Ninety patients with Sanders type Ⅱcalcaneal fractures enrolled in our hospital from September 2015 to September 2018 were selected and divided into control group and observation group according to different operation plans, with 45 cases in each group. The control group was treated with L-shaped incision anatomical plate internal fixation, and the observation group was treated with microincision locking plate internal fixation. The operation related indexes, pain scores, foot function recovery, calcaneus varus angle and complications were compared between the two groups. Results The intraoperative blood loss of the observation group was less than that of the control group, and the operation time and hospitalization time were shorter than those of the control group(P<0.05). The VAS scores of the observation group were lower than those of the control group at 1 day,1 week and 1 month after operation(P<0.05). The AOFAS score of the observation group at 3 months after operation were higher than that of the control group and before operation(P <0.05). There were no significant differences in calcaneus varus angle between the two groups before operation and at 3 months after operation(P>0.05);the calcaneus varus angle in the two groups decreased significantly at 3 months after operation(P<0.05). The total incidence of complications in the observation group was lower than that in the control group(P <0.05). Conclusion Micro-incision locking plate internal fixation on patients with Sanders type Ⅱ calcaneus fractures has the advantages of small trauma, short operation time, less complications and postoperative pain, which can promote postoperative recovery, improve the AOFAS score, and effectively put the calcaneus line back.
