摘　　要：目的探讨建立有限采样法(LSS)估算天疱疮患者口服霉酚酸酯(MMF)血药浓度-时间曲线下面积(area under the concentration-time curve,AUC)的模型。方法采集2016年10月~2018年12月收治的28例天疱疮患者口服MMF服药前和服药后0.5、1、2、4、6、8、12 h外周静脉血,用酶放大免疫测定技术(EMIT)检测血浆霉酚酸(MPA)浓度,用线性梯形法计算服药后0~12 h的MPA-AUC,用SPSS软件拟合估算MPA-AUC 0~12 h的回归方程及相关系数,并用预测误差评价模型准确性。结果28例天疱疮患者的MPA血药浓度-时间曲线形态基本相同;其中1点和2点拟合的复相关系数较低,3~5点拟合的复相关系数较高,共遴选出复相关系数较高的9个模型,其中模型3至模型9精准性递加。结论考虑可操作性和检测成本,推荐3点、4点的数学模型可用于临床估算天疱疮患者服用MMF后MPA-AUC 0~12 h,指导个体化给药。Objectives To investigate the establishment of area of limited sampling strategies for the estimation of Mycophenolic acid area under the concentration-time curve(AUC)in pemphigus patients.Methods The peripheral venous blood of 28 patients with pemphigus admitted into our hospital from October 2016 to December 2018 were collected before taking MMF and after taking MMF at 0.5,1,2,4,6,8 and 12 h.The concentration of mycophenolic acid(MPA)in plasma was measured by enzyme amplification immunoassay(EMIT).The MPA-AUC was calculated by linear trapezoid method at 0-12 h after administration.The regression equation and correlation coefficient of MPA-AUC 0-12 h were estimated by SPSS software,and the accuracy of the model was evaluated by prediction error.Results A total of 28 patients with pemphigus had the same shape of MPA blood concentration time curve.Among them,the complex correlation coefficient of 1 point and 2 point fitting was lower,and the complex correlation coefficient of 3-5 point fitting was higher.A total of 9 models with high complex correlation coefficient were selected,among which the accuracy of model 3 to model 9 was increased.Conclusion Considering the operability and cost of detection,it is recommended that the mathematical model of 3 and 4 points can be used to estimate the MPA-AUC 0-12 h after taking MMF in pemphigus patients.
