分段切除整形联合吻合器痔上黏膜环状切除术治疗环状混合痔的临床效果

摘　　要：目的探讨分段切除整形联合吻合器痔上黏膜环状切除术(PPH)治疗环状混合痔的临床效果。方法选取2016年2月~2019年2月我院收治的89例环状混合痔患者作为研究对象,根据抽签分组方式将患者分为观察组(45例)与对照组(44例)。观察组患者实施分段切除整形联合PPH治疗,对照组患者实施分段切除整形治疗。比较两组患者的手术指标、不同时间疼痛评分、肛门功能改善情况以及治疗效果。结果观察组患者的手术时间、住院时间、切口愈合时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组患者术后12 h、术后1周的疼痛评分低于对照组,差异有统计学意义(P<0.05);观察组患者的肛门功能改善情况优于对照组,差异有统计学意义(P<0.05)。观察组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论环状混合痔应用分段切除整形联合PPH治疗的临床效果十分显著,且安全性高,能够使其疼痛程度得以减轻,改善其肛门功能,值得研究和推广。Objective To investigate the clinical effect of segmental resection and plastic surgery combined with anastomat procedure for prolapse and hemorrhoid(PPH)in the treatment of circumferential mixed hemorrhoids.Methods Eighty-nine patients with circumferential mixed hemorrhoids treated in our hospital from February 2016 to February 2019 were selected as the research objects,and they were divided into observation group(45 cases)and control group(44 cases)according to the the method of drawing lots.The observation group received segmental resection and plastic surgery combined with PPH,and the control group received segmental resection and plastic surgery.Surgical indicators,pain scores at different times,improvement of anal function,and treatment effect were compared between the two groups.Results The operative time,hospitalization time and wound healing time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,with statistical differences(P<0.05).The pain scores of patients in the observation group were lower than those in the control group 12 h and 1 week after the surgery,the difference was statistically significant(P<0.05).The improvement of anal function in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical effect of circumferential mixed hemorrhoids treated with segmental resection and plastic surgery combined with PPH is very significant,and it is highly safe.It can reduce the pain and improve its anal function.It is worthy of research and promotion.
