改良Z成形术矫治内眦赘皮同期行重睑术效果研究

摘　　要：目的:比较Z成形术与改良Z成形术矫治内眦赘皮的疗效。方法:将笔者医院眼科2017年1月-2019年1月收治的120例内眦赘皮患者,随机分为观察组(n=60)和对照组(n=60)。对照组采用Z成形术,观察组采用改良Z成形术。对照组和观察组中内眦角<45°者分为A1、B1组,内眦角>55°者分为A2、B2组。比较两组患者的视觉模拟量表(VAS)评分、内眦间距(ICD)缩短率、睑裂长度与内眦间距比值(HLFL/ICD)达标率和瘢痕评分情况。结果:观察组患者术后VAS评分显著低于对照组,术后ICD缩短率、HLFL/ICD达标率显著高于对照组,差异均有统计学意义(P<0.05)。术后1周、1个月两组瘢痕评分比较差异无统计学意义(P>0.05);术后3个月观察组瘢痕评分显著低于对照组(P<0.05);A1组和B2组术后内眦角更符合标准值。结论:改良Z成形术矫治内眦赘皮可有效减少瘢痕,提高疗效,临床应用价值较高。Objective To compare the effects of Z-plasty and modified Z-plasty on epicanthus.Methods 120 patients with epicanthus were randomly divided into the observation group (n=60) and the control group (n=60).The control group was treated with Z-plasty,while the observation group was treated with modified Z-plasty.The control group and the observation group were divided into A1 and B1 groups if the angle of palpebral fissure was less than 45°,and A2 and B2 groups if the angle of palpebral fissure was more than 55°.The score of visual analogue scale(VAS),intercanthal distance(ICD)shortening rate,ratio of palpebral fissure length to intercanthal distance(HLFL/ICD)compliance rate and scar score of the two groups were compared.Results The VAS score of the patients in the observation group was significantly lower than that in the control group,the ICD shortening rate and the HLFL/ICD reaching rate in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in VSS score between the two groups at one week and one month after operation(P>0.05).The VSS score of the observation group was significantly lower than that of the control group 3 months after operation(P<0.05).The angle of inner canthus in group A1 and group B2 was more in line with the standard value.Conclusion Modified Z-plasty for correction of epicanthus can effectively reduce the scar and improve the curative effect,and has high clinical application value.
