摘 要:慢性阻塞性肺疾病是气道、肺部慢性炎症性疾病,引起炎症反应的原因多是感染,气道阻塞是感染反复发作的内在因素之一。参与炎性反应的因子相当繁多,参与炎症反应的信号通路也十分复杂,炎症反应的信号通路可能是单一的传导通路,也可能是复合的传导通路,通路之间可能存在相互激活、相互制约、相辅相成的网络交叉作用。中医药对慢性阻塞性肺疾病的气道、肺部炎症反应的作用机制相当广泛,有基因层面,有蛋白、因子层面等。研究表明,中医方药有抑制Toll样受体(toll-like receptor,TLR)信号通路的活性、抑制酪氨酸激酶/转录因子(janus kinase/signal transducers and activators of transcription,JAK-STAT)信号通路因子的转录、降低磷脂酰肌醇-3激酶/蛋白激酶B(phosphatidylinositol 3-hydroxy kinase/protein kinase B,PI3K/AKT)信号通路的传导、抑制丝裂原活化蛋白激酶(mitogen-activated protein kinase,MAPK)信号通路酶的激活、抑制转化生长因子β(transforming growth factor-β,TGF-β)/胞质蛋白(smads signaling pathway,Smads)信号通路因子的表达、抑制基质金属蛋白酶(matrix metalloproteinases,MMPs)信号通路蛋白的表达、抑制核因子-κB(nuclear factor-κB,NF-κB)的转导,同时对多个重信号通路进行调节等作用,从不同角度、不同程度对慢性阻塞性肺疾病的信号通路进行干预调控。Chronic obstructive pulmonary disease(COPD)is a chronic inflammatory disease of the airways and lungs.The cause of inflammation response is mostly infection,and airway obstruction is one of the internal factors of recurrent infection.There are many factors involved in the inflammatory response,and the signaling pathways involved in the inflammatory response are also very complex.The signaling pathways of the inflammatory response may be a single conduction pathway or a complex conduction pathway,and there may be a network interaction of mutual activation,restriction and complementarity between the pathways.Traditional Chinese medicines have a wide range of mechanisms of action on the inflammatory response of airway and pulmonary inflammatory response of COPD,including gene,protein and factor levels.Research shows TCM prescriptions have effects of inhibiting the activity of toll-like receptor(TLR)signaling pathway,inhibiting tyrosine kinase/transcription factor(JAK-STAT)signaling pathway gene transcription,reducing the conduction of phosphatidylinositol 3-hydroxy kinase/protein kinase B(PI3K/AKTkt)signaling pathway,inhibiting the activation of signaling pathway enzymes of mitogen-activated protein kinase(MAPK),inhibiting the expressions of transforming growth factor-β(TGF-β)or cytoplasmic protein(Smads)signaling pathway factor,inhibiting the protein expression of matrix metalloproteinases(MMPs)signal pathway,inhibiting the transfer of nuclear factor-κB(NF-κB),and simultaneously regulating multiple signaling pathways,and intervening and regulating the signaling pathways of COPD from different angles and degrees.
