祛风宣痹方联合孟鲁司特钠治疗过敏性鼻炎哮喘综合征临床研究

摘　　要：目的:探讨自拟祛风宣痹方联合孟鲁司特钠治疗过敏性鼻炎哮喘综合征(CARAS)的疗效及对血清Th1/Th2细胞因子的影响。方法:选取CARAS患儿108例,采用随机数字表法分为对照组和观察组,各54例。对照组给予布地奈德福莫特罗、孟鲁司特钠治疗,观察组在对照组基础上给予自拟祛风宣痹方治疗。比较两组临床疗效,治疗前后鼻炎及哮喘症状评分,血清TNF-β、IFN-γ、IL-4、IL-5指标水平变化。结果:治疗后两组中医症状积分均明显下降,且观察组比对照组下降更明显(P<0.05)。观察组临床控制率83.33%高于对照组62.96%(P<0.05),两组总有效率比较(观察组98.15%与对照组94.44%)差异无统计学意义(P>0.05);两组治疗前鼻炎、哮喘症状评分,血清TNF-β、IFN-γ、IL-4、IL-5比较差异无统计学意义(P>0.05);治疗后,两组TNF-β、IFN-γ均升高,IL-4、IL-5、鼻炎及哮喘症状评分均降低(P<0.05);观察组治疗后TNF-β、IFN-γ均高于对照组,IL-4、IL-5、鼻炎及哮喘症状评分均低于对照组(P<0.05)。治疗期间,两组患儿均未出现明显的不良反应情况(P>0.0.5)。结论:祛风宣痹方联合孟鲁司特钠可显著改善CARAS临床症状,调节Th1/Th2平衡,临床疗效确切。Objective:To investigate the efficacy of Qufeng Xuanbi decoction combined with montelukast sodium in the treatment of allergic rhinitis and asthma syndrome(CARAS)and its effect on serum Th1/Th2 cytokines.Methods:108 children with CARAS were selected.Randomly divided into control group and observation group,54 cases each.The control group was treated with budesonide,formoterol and montelukast sodium,and the observation group was treated with Qufeng Xuanbi decoction on the basis of the control group.The clinical effect,the score of rhinitis and asthma,the changes of serum TNF-β,IFN-γ,IL-4 and IL-5 were compared before and after treatment.Results:After treatment,the TCM symptom scores of the two groups decreased significantly,and the observation group decreased more significantly than the control group(P<0.05).The clinical control rate of the observation group was 83.33%higher than that of the control group(62.96%)(P<0.05).There was no significant difference in the total effective rate between the two groups(98.15%in the observation group and 94.44%in the control group)(P>0.05).There was no significant difference in the scores of rhinitis and asthma symptoms,serum TNF-β,IFN-γ,IL-4 and IL-5 between the two groups before and after treatment(P>0.05).The scores of TNF-βand IFN-γin the observation group were higher than those in the control group,and the scores of IL-4,IL-5,rhinitis and asthma were lower than those in the control group(P<0.05).During the treatment,there was no significant adverse reaction in the two groups(P>0.5).Conclusion:Qufeng Xuanbi decoction combined with montelukast sodium can significantly improve the clinical symptoms of CARAS,regulate the balance of Th1/Th2,have a definite clinical effect.
