益气活血汤与骨折消肿汤预防老年髋部骨折术后下肢深静脉血栓形成的效果比较

摘　　要：目的探讨益气活血汤与骨折消肿汤预防老年髋部骨折术后下肢深静脉血栓形成的效果。方法选取2017年1月~2018年10月在我院接受髋关节置换术治疗的60例老年髋部骨折患者作为研究对象,按照随机数字表法分为实验组(30例)与对照组(30例)。对照组术后采取骨折消肿汤治疗,实验组术后采取益气活血汤治疗。比较两组的下肢深静脉血栓发生率、不良反应发生率、血浆凝血酶原时间、纤维蛋白原、活化部分凝血酶时间、D-二聚体水平。结果实验组的下肢深静脉血栓发生率低于对照组,差异有统计学意义(P<0.05);实验组的不良反应总发生率低于对照组,差异有统计学意义(P<0.05);实验组的血浆凝血酶原时间、活化部分凝血酶时间长于对照组,纤维蛋白原、D-二聚体水平低于对照组,差异有统计学意义(P<0.05)。结论老年髋部骨折手术患者接受益气活血汤治疗,能够促进患者血液循环,降低下肢深静脉血栓的发生率,且不良反应较少,值得被推广、应用。Objective To explore the effect of Yiqi Huoxue Decoction and Fracture Detumescence Decoction in preventing deep venous thrombosis in lower limbs after hip fracture in the elderly.Methods Sixty elderly hip fracture patients who underwent hip replacement surgery in our hospital from January 2017 to October 2018 were selected as the research subjects and divided into experimental group(30 cases)and control group(30 cases)according to random number table method.The control group was treated with Fracture Detumescence Decoction after operation,and the experimental group was treated with Yiqi Huoxue Decoction after operation.The incidence of deep venous thrombosis of the lower limbs,incidence of adverse reactions,plasma prothrombin time,fibrinogen,activated partial thrombin time,D-dimer levels were compared.Results The incidence of lower limbs deep venous thrombosis in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05);the total incidence of adverse reactions in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05);The plasma prothrombin time and activated partial thrombin time were longer in the experimental group than in the control group,and the levels of fibrinogen and D-dimer were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Elderly hip fracture surgery patients receiving Yiqi Huoxue Decoction can promote blood circulation and reduce the incidence of deep venous thrombosis of lower limbs,and have fewer adverse reactions,which is worthy of promotion and application.
