摘　　要：目的:分析切开复位内固定复位和植骨治疗骨质疏松性肱骨近端骨折改善行为能力的效果。方法:选取2015年1月-2019年6月本院收治的62例骨质疏松性肱骨近端骨折患者,按治疗方法分为Ⅰ组(采用切开复位内固定治疗,n=30)与Ⅱ组(采用切开复位内固定结合同种异体骨植骨治疗,n=32)。对比两组骨折愈合情况、肩关节功能改善效果、行为能力评分、不良反应发生情况。结果:两组治疗前躯体生活自理表(physical self-maintenance scale,PSMS)与工具性日常生活活动能力(instrumental activity of daily living,IADL)量表评分比较,差异均无统计学意义(P>0.05);治疗后,Ⅱ组颈干角大小、PSMS及IADL量表评分均低于Ⅰ组,且骨折愈合时间更短,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:骨质疏松性肱骨近端骨折行切开复位内固定联合植骨治疗可以有效提高愈合效果,纠正肩关节功能,从而促进行为能力改善。Objective:To analyze the effect of open reduction internal fixation combined with allogeneic bone grafting on improving the behavior ability of patients with osteoporotic proximal humeral fractures.Method:A total of 62 patients with osteoporotic proximal humeral fractures admitted to our hospital from January 2015 to June 2019 were divided into Ⅰ group (open reduction internal fixation,n=30) and Ⅱ group (open reduction internal fixation combined with allogeneic bone grafting,n=32).The fracture healing,shoulder joint function improvement effect,behavior ability score and incidence of adverse reactions of two groups were compared.Result:There were no significant differences in physiscal self-maintenance scale (PSMS) and instrumental activity of daily living (IADL) scale scores between two groups before treatment (P>0.05).After treatment,the humeral neck stem angle,PSMS and IADL scale scores in Ⅱ group were lower than those in Ⅰ group,and fracture healing time was shorter,there were significant differences (P<0.05).There was no significant difference between two groups in the incidence of adverse reactions (P>0.05).Conclusion:Osteoporotic proximal humeral fractures treated with open reduction internal fixation combined with allogeneic bone grafting can effectively improve the healing effect,correct the shoulder joint function,and promote the improvement of behavior.
