摘 要:目的:对比分析CT尿路造影(computer tomography urography,CTU)的常规扫描和改良扫描方案的辐射剂量及对尿路结构显示准确性的差异,以探讨改良扫描方案的价值。方法:前瞻性连续选取疑诊尿路疾患、随机进行CTU分段注射和常规扫描、且CTU诊断阴性的80例患者的资料。实验组40例,采用分次团注三期CTU检查;对照组40例,采用传统四期CTU检查。由两名主治以上影像科医师对CTU的图像质量进行评估,内容包含两种方法的肾盂和各端输尿管管腔扩张度比较、肾盂和各端输尿管管腔内CT值比较,并对比分析两种方式CTU对患者的辐射剂量的差异。结果:肾盂及输尿管上、中和下端管腔扩张度和相对应各段CT值数据比较差异无统计学意义(P>0.05);对患者所受辐射剂量的差异有统计学意义(P<0.05)。结论:改良的CTU方法在输尿管管腔扩张程度上无明显差异,但能减少患者受到的辐射剂量,且基本不影响对疾病的诊断。Objective:To compare the difference of radiation dose and its diagnostic accuracy in routine scan and improved scan of computer tomography urography(CTU),and to explore the application value of modified scan.Methods:80 cases were admitted to our hospital with CTU examination,which were suspected of urinary tract diseases,and they were randomly divided into experimental group and control led group.In the experimental group,40 patients were examed by split-blous 3 phases,while 40 cases in the control group were examined by the traditional 4 phase CTU.The imaging quality of CTU was evaluated by two experienced radiologists including the dilation of the renal pelvis and ureterand CT value of the lumendiameter,lumen concentrations.Results:There was no statistically significant in the dilation of the renal pelvis andureter of the upper,middle and lower ureters and CT value of the corresponding segments between the 2 groups(P>0.05),while there was statistically significant in dose between the 2 groups(P<0.05).Conclusion:The improved CTU is as good as the traditional CTU in evaluatingthe degree of dilatation of the ureter,it can also reduce the radiation dose with a good imaging quality.
